目的 探讨滤泡性树突状细胞肉瘤的病理形态学和免疫表型特点.方法 对4例滤泡性树突状细胞肉瘤的石蜡标本进行病理学和免疫组化染色观察.结果 滤泡性树突状细胞肉瘤的病理特点为肿瘤细胞呈巢片状、旋涡状或束状排列,瘤细胞边界不清,胞质丰富,淡嗜酸性,常见多核或单核瘤巨细胞,胞核染色空淡,间质常有多少不等的淋巴细胞浸润.免疫组化:瘤细胞CD21、CD35和clusterin(+),部分肿瘤细胞EGFR、S-100、CD68、EMA和vimentin(+),CK和CD1a(-),Ki-67表达程度不一.结论 滤泡性树突状细胞肉瘤形态学变化较大,确诊需要依靠详细的病理形态观察和多种免疫组化染色.

Objective To observe the clinicopathologic features and immunophenotypic characteristics in four cases of follicular dendritic cell sarcoma (FDCS). Methods The morphologic features and immunohistochemical markers were studied in four cases of FDCS. Results The tumor cells were arranged in nest and whorled or sheet patterns. The tumor cells with vesicular nuclei were characterized by abundant and scant eosinophilic cytoplasm with indistinct cell bordes, dispersed by the small lymphocytes. Immunohistochemically, tumor cells were positive for CD21, CD35, clusterin, and weakly positive for EGFR, S-100, CD68, EMA and vimentin, but negative for CK and CD1a. Ki-67 showed a variable expression among cases. Conclusion The morphologic change of FDCS overlaps with many other spindle cell neoplasms. The diagnosis should be based on the combined morphological and immunophenotypical analysis.
