Extrahepatic portal hypertension in children is not uncommon.The majority of cases of portal hypertension occurs secondary to extrahepatic block caused by portal vein thrombosis and umbilical infection.There is evidence of a congenital basis in portal vein obstruction and vascular anomalies.During a period of 27 years,1958-1985,17 children (10 males,7 females) were treated for extrahepatic portal hypertension in our institute,14 of them by certain kind of portocaval shunt and 11 of them were followed-up for 4-20 years.Two of these patients had recurrent episodes of bleeding but were readily controlled.Clinically,the children were normal in development.There were no instance of liver enlargement and liver function impairment.The first symptom was either acute upper gastraintes-tinal bleeding or splenomegaly.It is our opinion that results of shunt operations were satisfactory,but shunt operation should not be done as an emergency procedure.
