目的探讨不同转位皮瓣联合异体巩膜植入在外伤性下睑缺损的临床效果.方法选择因外伤致下睑全层缺损程度大于1/2,导致眼睑闭合不全,下睑呈退缩状态＞2mm的8例,采用不同部位的转位皮瓣代替下睑前层,异体巩膜植入和分离的残留穹窿结膜代替下睑后层,从而矫正缺损的下睑.结果随访12 ～24个月,全部病例异体巩膜血管化,无溶解和排斥现象,皮肤活性好,下睑缘位于角膜下缘或稍高,无巩膜暴露,l例轻度下睑外翻,外观形态基本正常.结论对于外伤所致大于1/2的下睑全层缺损,异体巩膜植入联合转位皮瓣治疗是一种有效方法.
목적 탐토불동전위피판연합이체공막식입재외상성하검결손적림상효과.방법 선택인외상치하검전층결손정도대우1/2,도치안검폐합불전,하검정퇴축상태＞2mm적8례,채용불동부위적전위피판대체하검전층,이체공막식입화분리적잔류궁륭결막대체하검후층,종이교정결손적하검.결과 수방12 ～24개월,전부병례이체공막혈관화,무용해화배척현상,피부활성호,하검연위우각막하연혹초고,무공막폭로,l례경도하검외번,외관형태기본정상.결론 대우외상소치대우1/2적하검전층결손,이체공막식입연합전위피판치료시일충유효방법.
Objective To investigate the clinical effect of different transplantation skin flap combined with allogenic scleral graft to treat traumatic defect of lower eyliid.Methods 8 cases with full-thickness traumatic defect of lower eyelid of which more than 50％ were defective and the retraction were more than 2mm due to hypophasis were selected.The defective lower eyelids were corrected by different site transplantation skin flap instead of anterior layer,allogenic sclera and dissociated remaining fornical conjunction instead of posterior layer.Results Following up for 12 to 24 months,the scleral graft were vaculized and no dissolving or rejection were found.The dermatal activity were well.Limbic palpebrales inferior were located on or mild higher than comeal inferior edge.No scleral exposure were found.1 case was mild ectropion while the others′outlooks were normal.Conclusion Transplantation skin flap combined with allogenic scleral graft is effective to full-thickness defect lower eyelids of which more than 50％ is defective resulted from trauma
