目的 探讨准分子激光原位角膜磨镶术(LASIK)术前对周边视网膜变性干预性激光光凝的方法和疗效.方法 LASIK术前对近视眼常规行直接检眼镜、三面镜和间接检眼镜检查,对合并有明显的周边视网膜变性的近视眼行532 nm激光干预性光凝治疗.在变性区外光凝2~3排,光斑直径200~300 μn,时间0.15~0.20 s,光斑间距1.0~1.5个光斑,出现三级光斑为准.结果 197例(231眼)视网膜变性,其中格子样变性163眼(70.56%),霜样变性64眼(27.71%),囊样变性4眼(1.73%).激光光凝术后,视网膜变性及干性裂孔区封闭良好,色素斑形成明显.LASIK术后随访6~18个月,无发生视网膜脱离者.结论 LASIK术前对周边部视网膜变性进行干预性光凝,是预防LASIK术后发生视网膜脱离的重要方法.

Objective To evaluate the therapeutic effects of intervenient laser photocoagulation for myopia patients with peripheral retinal degeneration before LASIK operation. Methods Myopia patients underwent conventional direct and indirect ophthalmoscopy and Goldman three mirror contact lens examination. The patients that were confirmed to have peripheral retinal degeneration were treated with 532 nm laser photocoagulation before LASIK operation. 2 - 3 rows of laser photocoagulation were done outside the degeneration area. The spot diameter was 200 - 300 μm shoting time was 0. 15 - 0. 2 s, interval spacing was 1 -1.5 spot, tertiary light spot manifested. Results 197 retinal degeneration patients (231 eyes)were found,including lattice degeneration in 163 eyes ( 70.56% ), frosted degeneration in 64 eyes ( 27.71% ), cystoid degeneration in 4 eyes ( 1.73% ). The retinal holes were sealed well and retinal condition was good after laser photocoagulation. Pigment spots were obviously formed. The patients were followed up for 6 - 18 months after LASIK, and no rhegmatogenous retinal detachment occurred. Conclusions Intervenient photocoagulation before LASIK for peripheral retinal degeneration was an important method to prevent retinal detachment after LASIK.
