目的 探讨腰骶部囊肿的临床表现、诊断及治疗方法.方法 回顾分析18例经手术证实的腰骶部囊肿患者的临床特点及术中的病理特点.18例中神经根囊肿13例,其中7例和硬脊膜相通的患者采用囊肿切除,硬脊膜修复,6例和硬脊膜不相通的患者采用囊肿切除;硬脊膜外囊肿4例,直接切除;椎间盘手术后继发硬脊膜囊肿1例切除囊肿并修复硬脊膜.结果 本组患者均采取手术治疗,其中15例得到6～186个月的随访,平均43.2个月,15例患者疗效优良,手术后腰腿疼痛等症状消失.结论 腰骶部囊肿是引起腰腿疼痛的病因之一,诊断依靠典型的症状、体征及脊髓造影CT、核磁共振等检查.不同囊肿手术的方式不同,手术效果良好.

Objective To investigate the clinical manifestations,diagnosis and treatment of lumbosacral cyst.Methods Eighteen lumbosacral cyst patients were confirmed by operation.Among them,13 cases were nerve root cyst,including 7 cases connected with endorhachis were treated with cyst excision and endorhachis repair;six case;endorhachis not connected with endorhachis were treated with cyst excision.Four cases of epidural cyst were treated with direct resection.One case of secondary spinal epidural cyst after disc surgery were treated with cyst excision and endorhachis repair.Results Fifteen cases had 6 to 186 months'follow-up,average 43.2 months.All of the 15 patients of an excellent effect without waist and leg pain after operation.Conclusion Lumbosacral cyst is one of the causes for waist and leg lumbosacral pain.The diagnosis relies on typical symptoms,signs and CTM,MRI examinations,which needs to be distinguished with lumbosacral tumor.Different cysts need different operations which can bring excellent effects.
