目的 探讨多波长氪激光治疗视网膜分支静脉阻塞(BRVO)合并黄斑囊样水肿的临床疗效.方法 经眼底荧光血管造影(FFA)确诊为视网膜分支静脉阻塞合并黄斑囊样水肿的患者85例85只眼,使用多波长氪离子激光治疗仪进行早期视网膜光凝治疗,光斑反应为Ⅲ级.术后连续随访12个月,对比分析视网膜光凝前及光凝后1个月、3个月、6个月、9个月及12个月的视力、眼底荧光血管造影、视网膜光学相干断层扫描(OCT)的变化情况.结果 与治疗前相比,激光术后1个月,3个月,6个月,9个月及12个月视力提高者分别为41例(48.24%),62例(72.95%),78例(91.76%),76例(89.41%),80例(94.12%);术后1个月,3个月,6个月,9个月及12个月眼底荧光血管造影示治愈的分别为62例(72.94%),73例(85.88%),77例(90.59%),81例(95.29%),82例(96.47%);治疗前黄斑区平均视网膜神经上皮层厚度为(607.89±176.31)μm;治疗后1个月、3个月、6个月、9个月及12个月的平均厚度分别为(502.72±176.32)μm、(420.15±152.40)μm、(352.13±145.21)μm、(348.21 ±138.56)μm、(325.82±127.80)μm.治疗前与治疗后不同时期的视网膜神经上皮层厚度进行t检验,P值均<0.05,差异有统计学意义.结论 氪激光对视网膜分支静脉阻塞合并黄斑囊样水肿的激光治疗有显著疗效,术后6个月病情基本稳定,效果明显.

Objective To evaluate the curative effect of laser photocoagulation in the treatment of branch retinal vein occlusion (BRVO) and cystoids macular edema.Methods A total of 85 patients (85 eyes) with branch retinal vein occlusion (BRVO) and cystoids macular edema diagnosed by fundus fluorescein angiography (FFA) were treated by laser photocoagulation with 3 class spot reaction.The period of follow-up was 12 months.The visual acuity, FFA, optical coherence tomography (OCT) were analyzed comparatively before photocoagulation and 1 month, 3 months, 6 months, 9 months and 12 months after photocoagulation.Results The visual acuity after photocoagulation of 1 month, 3 months, 6 months, 9 months and 12 months were improved 41 (48.24%) eyes, 62 (72.95%), 78 (91.76%), 76 (89.41%) and 80 (94.12%) respectively.The cured patients showed by FFA after photocoagulation of 1 month, 3 months, 6 months, 9 months and 12 months were 62(72.94%) eyes, 73 (85.88%), 77 (90.59%), 81 (95.29%) and 82 (96.47%) respectively.The thickness of RNL before treatment was (607.89± 176.31) um, while which at 1 month, 3 months, 6 months, 9 months and 12months after treatment were (502.72± 176.32) m, (420.15± 152.40) m, (352.13± 145.21) m, (348.21±138.56) m, and (325.82± 127.80) m respectively.Compared the RNL of the different periods of the post treatment and before treatment, the difference was significance (P <0.05).Conclusions Krypton laser photocoagulation is obviously effective on treatment of branch retinal vein occlusion and cystoids macular edema,and it is stable 6 months after laser photocoagulation.
