目的 评价安氏Ⅰ类错(牙合)矫正治疗中个体化弓丝和横腭弓联合应用的支抗控制效果.方法 选择18岁8个月至25岁6个月安氏Ⅰ类错(牙合)女性患者24例,分析个体化弓丝和横腭弓联合应用加强支抗治疗前后的X线头影测量值的变化.结果 Ptm-6与So-6AP虽有增大(P<0.05),但支抗磨牙的前移量不到拔牙间隙的1/5;6-PP、MP-FH及ANS-Me治疗前后均无显著性改变(P>0.05),支抗磨牙在垂直方向上也得到了有效的控制;水平方向上无支抗磨牙的舌向倾斜及近中舌向扭转.结论 个体化弓丝和横腭弓联合应用能有效地稳定支抗磨牙.

Objective This study was designed to examine the effect of the custom-bent archwire in enforcing molar anchorage during canine and /or incisal retraction.Methods 24 female adult patients diagnosed with Class Ⅰ with severe crowding or Class Ⅰwith moderate crowding and proclined upper incisors,were included into this study.The age of the patients ranged from 18 years 8 months to 25 years 5 months.As required by treatment plan, hte patients were extracted uppe &lower premolars to relieve crowding and retract frontal teeth. The upper starting archwire of 0.018ss was custom-bent with its first order bends fitting to the irregularities of the upper dentition,so that it was engaged into the brackets passively.The transpalatal Arch(TPA) was also placed transversely connecting bilateral molars.Canine disalzation was implemented firstly using ligature wire lace-back then active power-chain elastics.Study models and cephalograms were taken before and after orthodontic treatment and the parameters,namely,Ptm-6,S0-6AP and 6-PP were measured to indic ate the displacement of the molars.Results (1)Althouth the increase in Ptm-6 and So-6AP was significant(P<0.05),a molar repositioning less than 1/5 of the extraction space.(2) There were no significant changes in 6-PP,MP-FH and ANS-Me(P>0.05),indicating a good control of the molars even in vertical dimension.Conclusions A combined use of custom-bent archwire with TPA enhances molar anchorage.
