目的探讨急性动脉栓塞外科治疗的体会.方法回顾性总结2005年11月至2008年9月间22例急性肢体动脉栓塞患者的临床资料,22例23条肢体均采用外科手术加抗凝治疗.结果死亡1例,肢体存活20例,取栓加截肢1例,残端愈合良好出院.1例骨筋膜室综合征切开减压.随访17例成功保肢者2～18个月,1例术后3个月心肌梗死死亡,2例因再栓塞再手术治愈.4例有慢性缺血表现.余10例效果满意.结论早期诊断,及时外科手术是治疗肢体动脉栓塞的有效方法,取栓术中注重Fogarty取栓导管的操作技巧,必要的手术及药物治疗对保存肢体更为有效.

Objective To investigate surgical treatment for acute arterial embolism of extremities. Methods The clinical data of 23 limbs in 22 cases with acute arterial embolism of extremities in our unit between November 2005 and September 2008 were analyzed retro-spectively. Twenty-two cases were treated by artefiotomy of femoral or brachial artery and embolectomy with Fogarty catheter in addition to anti-coagulation. Results The extremities of 20 eases were saved, 1 case died, amputation and embolectomy was performed in 1 case and fasciectomy in 1 case. Finally, 17 cases had saved ed limbs were followed up for 2 to 18 months ,1 ease died of acute myocardial infarction 3 months after the operation, 4 cases had chronic ischemia, 2 cases were re -operated because of recurrent embolism, other 10 cases recov-ered. Conclusion Early diagnosis and timely surgical intervention are keys tothe prognosis of acute arterial embolism of extremities. Pay at-tention to operative skill of Fogarty balloon catheter in embolectomy, operation and drug management were helpful in limb survival, n case of necessity.
