目的 报告同指指背岛状皮瓣携带指神经背侧支一期修复手指皮肤缺损合并神经缺损的临床效果.方法 对16例18指手指皮肤缺损伴一侧神经断裂缺损应用携带指神经背侧支同指指背岛状皮瓣修复,重建手指感觉.皮肤缺损面积为10mm×7mm~24mm×20mm.神经缺损长度9~22mm.结果 16例18指皮瓣全部存活,随访5~25个月,手指功能及外观恢复满意,伤侧指腹感觉恢复至S3+,两点分辨觉恢复至4~7mm.结论 同指指背岛状皮瓣携带指神经背侧支修复手指皮肤缺损合并神经缺损,能同时完成皮肤覆盖和神经缺损的修复,术后效果满意,是治疗手指皮肤合并神经缺损并重建感觉较为理想的方法.
目的 報告同指指揹島狀皮瓣攜帶指神經揹側支一期脩複手指皮膚缺損閤併神經缺損的臨床效果.方法 對16例18指手指皮膚缺損伴一側神經斷裂缺損應用攜帶指神經揹側支同指指揹島狀皮瓣脩複,重建手指感覺.皮膚缺損麵積為10mm×7mm~24mm×20mm.神經缺損長度9~22mm.結果 16例18指皮瓣全部存活,隨訪5~25箇月,手指功能及外觀恢複滿意,傷側指腹感覺恢複至S3+,兩點分辨覺恢複至4~7mm.結論 同指指揹島狀皮瓣攜帶指神經揹側支脩複手指皮膚缺損閤併神經缺損,能同時完成皮膚覆蓋和神經缺損的脩複,術後效果滿意,是治療手指皮膚閤併神經缺損併重建感覺較為理想的方法.
목적 보고동지지배도상피판휴대지신경배측지일기수복수지피부결손합병신경결손적림상효과.방법 대16례18지수지피부결손반일측신경단렬결손응용휴대지신경배측지동지지배도상피판수복,중건수지감각.피부결손면적위10mm×7mm~24mm×20mm.신경결손장도9~22mm.결과 16례18지피판전부존활,수방5~25개월,수지공능급외관회복만의,상측지복감각회복지S3+,량점분변각회복지4~7mm.결론 동지지배도상피판휴대지신경배측지수복수지피부결손합병신경결손,능동시완성피부복개화신경결손적수복,술후효과만의,시치료수지피부합병신경결손병중건감각교위이상적방법.
Objective To report the treatment outcomes of repairing soft tissue and nerve defect of the fingers using dorsal island flap carrying dorsal cutaneous branch of the proper digital nerve. Methods The skin and nerve defects in 18 fingers of 16 patients were repaired by dorsal island flap carrying dorsal cutaneous branch of the proper digital nerve. The size of the flaps ranged from 10 mm × 7 mm to 24 mm × 20 mm. The length of the nerve defect ranged from 9 to 22 mm. Results Postoperatively all the flaps in the 18 fingers of 16 patients survived. The average follow-up was 5 to 25 months. The appearance and function of the involved fingers were satisfactory. Sensation of the finger pulp returned to S3+ and two-point discrimination ranged between 4 to 7 mm. Conclusion Using dorsal island flap carrying dorsal cutaneous branch of the proper digital nerve can achieve simultaneous repair of both skin and nerve defects. The treatment outcomes were satisfactory. It is an optimal procedure for repairing combined soft tissue and nerve defect of the fingers.
