目的探讨能否利用损伤的神经远端延长法修复自身缺损。方法观察A组(即刻快速延长10%)、B1组、B2组(神经损伤2周后再行延长,延长率分别为10%、20%)神经耐受程度和恢复情况。结果 A组平均延长(10.00±1.71)%,长度为(4.00±0.74)mm,神经传导速度恢复率 ( 75.71 ± 19.05)%、有髓神经纤维(NNF)再生率为( 72.60±15.76)%;B1分别为(10.50±1.12)%、(4.20±0.44)mm、(82.08± 4.76)%、 (76.22 ± 8.60)%;B2组分别为(21.40±4.03)%、(8.50±1.61)mm、(70.87± 20.86)% 、( 64.67 ±12.42)%。即刻延长10%和损伤2周后快速延长10%、20%都有一定功能的恢复。结论变性周围神经的即刻延长在10%延长范围内耐受性好,而延长20%仍有再生,可以利用周围神经损伤远端即刻延长的方法来修复神经缺损。

목적탐토능부이용손상적신경원단연장법수복자신결손。방법관찰A조(즉각쾌속연장10%)、B1조、B2조(신경손상2주후재행연장,연장솔분별위10%、20%)신경내수정도화회복정황。결과 A조평균연장(10.00±1.71)%,장도위(4.00±0.74)mm,신경전도속도회복솔 ( 75.71 ± 19.05)%、유수신경섬유(NNF)재생솔위( 72.60±15.76)%;B1분별위(10.50±1.12)%、(4.20±0.44)mm、(82.08± 4.76)%、 (76.22 ± 8.60)%;B2조분별위(21.40±4.03)%、(8.50±1.61)mm、(70.87± 20.86)% 、( 64.67 ±12.42)%。즉각연장10%화손상2주후쾌속연장10%、20%도유일정공능적회복。결론변성주위신경적즉각연장재10%연장범위내내수성호,이연장20%잉유재생,가이이용주위신경손상원단즉각연장적방법래수복신경결손。
Objective To investigate the feasibility of repairing defect by segment expansion of the distal injured nerve. Methods A rabbit median nerve injury model was employed. Twenty-four rabbits weighing 2.5~3.0 kg were used for stretching their wallerian degenerating median nerves with TE. Rabbits were divided into three groups: group A (the distal end of nerves were expanded about 10% immediately after injuries), group B1 (the distal end of nerves were expanded about 10% two weeks later after injuries), group B2 (the distal end of nerves were expanded about 20% two weeks later after injuries). All animals were evaluated by electrophysiological histological methods. Results The nerves were increased in length. The mean elongation was (4.00±0.74) mm with an elongation rate of (10.00±1.71)%, recovery rate of nerve conduction speed (75.71±19.05)% and the regenerating rate of NNF (72.60±15.76)% in group A; (4.20±0.44) mm with an elongation rate of (10.50±1.12)%, recovery rate of nerve conduction speed (82.08±4.76)% and the regenerating rate of NNF (76.22±8.60)% in group B1; (8.50±1.61) mm with an elongation rate of (21.40±4.03)%, recovery rate of nerve conduction speed (70.87±20.86)% and the regenerating rate of NNF (64.67±12.42)% in group B2. The data showed good function recovery. Conclusion The ideal range for rapid expansion of the degenerating nerve was 10%. Regeneration still existed in expansion of 20%. The method for rapid expansion of the distal injured peripheral nerve could be used to repair nerve defect.
