目的　总结我院1989～1998年治疗侵及胰周重要血管的胰腺癌治疗经验。方法　回顾性分析胰腺、受侵血管联合切除术治疗晚期胰腺癌的手术方法和近期疗效。结果　侵及胰周重要血管的胰腺癌共37例，其中13例行胰头癌及受侵血管联合切除术，切除血管段为0.5～4.0 cm不等，其中2例血管直接吻合、10例修补缝合、1例行人造血管移植。血流阻断时间20～50 min，术后病人无并发症，无院内死亡。结论　胰头癌和受侵胰周重要血管联合切除术不仅可完整切除病灶，达到相对根治目的，且手术创伤不大，是治疗概念上部分无法切除的胰头癌的安全、可行的术式。
목적　총결아원1989～1998년치료침급이주중요혈관적이선암치료경험。방법　회고성분석이선、수침혈관연합절제술치료만기이선암적수술방법화근기료효。결과　침급이주중요혈관적이선암공37례，기중13례행이두암급수침혈관연합절제술，절제혈관단위0.5～4.0 cm불등，기중2례혈관직접문합、10례수보봉합、1례행인조혈관이식。혈류조단시간20～50 min，술후병인무병발증，무원내사망。결론　이두암화수침이주중요혈관연합절제술불부가완정절제병조，체도상대근치목적，차수술창상불대，시치료개념상부분무법절제적이두암적안전、가행적술식。
Objective　To summarize experience in treatment of pancreatic cancer invading peripancreatic vessels. Methods 　 The data of 37 cases of pancreatic cancer invading peripancreatic vessels treated in our department from 1989 to 1998 were retrospectively analyzed to investigate operative methods and their short-term therapeutic effects. Results 　 Of the patients, 13 were treated with the combined resection. The length of resected vessel was 0.5-4.0 cm and duration of blood flow stopping 20-50 min. No postoperative complications and death in hospital occurred. Conclusions 　 Extended pancreatic-duodenal resection is a safe and effective treatment for patients with pancreatic cancer invading peripancreatic vessels.
