目的探讨胰腺实性假乳头状瘤(SPTP)细针吸取细胞块的诊断价值.方法回顾性分析胰腺实性假乳头状瘤患者8例,术前均行B超引导细针穿刺(fine needle aspiration guided on ultrasound B,US-FNA)细胞学涂片及细胞块检查,细胞切片免疫组化染色,并与术后病理诊断对照.结果细胞块结合免疫组化诊断结果与术后病理结果吻合,准确率达100%(8/8),细胞学涂片诊断符合率为75%(6/8),均未出现出血、感染等并发症.结论 B超引导下的FNA是术前诊断SPTP的一种安全有效的检查方法,制作细胞块及免疫组化检查有利于诊断的进一步完善,为确定治疗方案提供了依据,有较高的临床应用价值.

Objective To investigate the diagnostic value of the fine needle aspiration of cell block on solid pseudopapillary tumor of the pancreas (SPTP). Methods Eight SPTP patients treated in our hospital were tested with smear cytology,cell block and immunohistochemistry by fine needle aspiration guided by ultrasound B,and the diagnosis were compared with the correct diagnosis made by operative exploration.Results The diagnosis of cell block and immunohistochemistry was consistant with the the final diagnosis available,the accuracy reached 100%(8/8).The accuracy of smear cytology was 75%(6/8).There was no haemorrhage,infection occured.Conclusions FNA guided by ultrasound B is a safe and effective approach for the diagnosis of solid pseudopapillary tumor of the pancreas.Cell block and immunohistochemistry test can further improve the diagnosis, which can provide evidences for the chinics to make therapeutic plans,the method definitely has important clinical values.
