目的探讨带虹膜隔人工晶状体植入术在外伤性虹膜缺失无品状体眼的安伞性及有效性.方法 2004年3月至2008年3月30例30只眼外伤性晶状体病变伴有虹膜缺失的患者.给予植入Morcher公司生产的带虹膜隔人工晶状体,观察术后疗效.所有患者均为Ⅱ期人工晶状体植入,随访6个月至2年.结果 24只眼(80%)的视力有较明显改善,5只眼的术后视力有所增加;另外有1只眼视力没有改变.所有患者对手术的美容效果都表示满意且畏光症状明显减轻或消失.术后有3只眼出现周边局部脉络膜浅脱离,经应用激素后治愈;6只眼术后前房反应较重;6只眼有眼压升高,经治疗后恢复正常.在以后的随访中未发现人工晶状体的移位以及视网膜脱离等并发症的发生.结论外伤性无晶状体眼联合虹膜大部或完伞缺失患者植入带虹膜隔人工晶状体可以明显改变屈光状态,减轻畏光,并起剑美容作用.本研究结果显示带虹膜隔人工晶状体是安全、有效的,值得临床应用.
목적 탐토대홍막격인공정상체식입술재외상성홍막결실무품상체안적안산성급유효성.방법 2004년3월지2008년3월30례30지안외상성정상체병변반유홍막결실적환자.급여식입Morcher공사생산적대홍막격인공정상체,관찰술후료효.소유환자균위Ⅱ기인공정상체식입,수방6개월지2년.결과 24지안(80%)적시력유교명현개선,5지안적술후시력유소증가;령외유1지안시력몰유개변.소유환자대수술적미용효과도표시만의차외광증상명현감경혹소실.술후유3지안출현주변국부맥락막천탈리,경응용격소후치유;6지안술후전방반응교중;6지안유안압승고,경치료후회복정상.재이후적수방중미발현인공정상체적이위이급시망막탈리등병발증적발생.결론 외상성무정상체안연합홍막대부혹완산결실환자식입대홍막격인공정상체가이명현개변굴광상태,감경외광,병기검미용작용.본연구결과현시대홍막격인공정상체시안전、유효적,치득림상응용.
Objective To evaluate the efficacy and safety of the black iris-diaphragm intraocular lens (IOLs)implantation in management oftranmatic aniridia and aphakia.Methods The black iris-diaphragm IOLs produced by Morcher company(Germany)were applied in secondary IOL implantation in 30 cases of severe traumatic aniridia and aphakia during March 2004,to 2008,followed up for 6 to 24 months for the postoperative curative effect.Results Most of the postoperative vision was improved.All cases were satisfied with the cosmetic outcomes.Photophobia lessened or disappeared.A few of the cases appeared choroidal detachment or ocular hypertension,but recovered after treatment.No black iris-diaphragm IOLs decentration and no new retinal detachments were seen during the follow-up period.Conclusions The black iris-diaphragm IOLs implantation is effective for the patients of traumatic aniridia and aphakia,which can improve the eyesight and decrease or eliminate the symptom of photophobia,all the same time have the effect of cosmetic.The study shows that the black iris-diaphragm IOLs implantation is safe and effective.
