目的 探讨2型糖尿病患者脉搏波速度的变化及其影响因素.方法 选择2型糖尿病患者(T2DM组)48例和健康体检者(NC组)22名,进行颈-股动脉脉搏波速度(CF-PWV)、颈-桡动脉脉搏波速度(CR-PWV)、颈-足背动脉脉搏波速度(CD-PWV)的测量.结果 2型糖尿病患者CF-PWV高于NC组;伴有合并症或/和并发症者CF-PWV升高更为显著,年龄、SBP、脉压差、糖尿病病程、尿白蛋白排泄率(UAER)、眼底病变与CF-PWV呈正相关,其中年龄和UAER是PWV的独立预测因素.结论 2型糖尿病中心动脉弹性减低,年龄、SBP、脉压差、糖尿病病程、UAER、眼底病变与中心动脉弹性减低有关.

목적 탐토2형당뇨병환자맥박파속도적변화급기영향인소.방법 선택2형당뇨병환자(T2DM조)48례화건강체검자(NC조)22명,진행경-고동맥맥박파속도(CF-PWV)、경-뇨동맥맥박파속도(CR-PWV)、경-족배동맥맥박파속도(CD-PWV)적측량.결과 2형당뇨병환자CF-PWV고우NC조;반유합병증혹/화병발증자CF-PWV승고경위현저,년령、SBP、맥압차、당뇨병병정、뇨백단백배설솔(UAER)、안저병변여CF-PWV정정상관,기중년령화UAER시PWV적독립예측인소.결론 2형당뇨병중심동맥탄성감저,년령、SBP、맥압차、당뇨병병정、UAER、안저병변여중심동맥탄성감저유관.
Objective To study the changes of pulse wave velocity(PWV)and its influential factors in the patients with type 2 diabetes. Methods 48 diabetic patients and 22 healthy people were enrolled in this study.Automatic PWV measuring system was applied to examine carotid-femoral PWV (CF-PWV),carotid-radial PWV (CR-PWV) and carotid-dorsum of foot PWV(CD-PWV). Results Compared with the control group, CF-PWV of the diabetic patients was significantly increased(P<0.05), and was correlated with age, systolic blood pressure, pulse pressure, diabetic course, UAER and diabetic retinopathy(r=0.57,0.38,0.51,0.44,0.449,0.319;all P<0.05 respectively), among which age(P<0.05) and UAER(P<0.05) were independent predictors of CF-PWV.The CD-PWV was increased only in the patients with complications,and no statistical difference in CR-PWV was showed among the groups. Conclusions The elasticity of central elastic large artery, which is associated with age, systolic blood pressure, pulse pressure, diabetic course, UAER and diabetic retinopathy, is decreased in type 2 diabetics.
