目的 建立百草枯中毒急性肺损伤模型,观察低分子肝素钙联合阿司匹林治疗百草枯中毒急性肺损伤的疗效.方法 120只成年雄性Wistar大鼠,随机分为3组,百草枯染毒组一次性给予百草枯50 mg/kg灌胃;抗凝治疗组一次性给予百草枯50 mg/kg灌胃,再给予低分子肝素钙68 U/kg,2次/d,皮下注射,同时给予阿司匹林1.67 mg/kg灌胃,1次/d,给药至动物处死前;对照组给予等量生理盐水灌胃.染毒组、治疗组和对照组分别于实验开始第3、7、14、21天分批处死动物,留取静脉血、肺组织待检.静脉血检测血浆凝血酶原时间(PT),活化部分凝血活酶时间(APTT)、血浆纤维蛋白原(Fib)、凝血酶时间(TT)、D-二聚体(D-dimer),肺组织检测羟脯氨酸,同时留取部分肺组织做病理切片,行HE和Masson染色后光学显微镜下观察.结果 染毒后3、7、14、21d,治疗组和染毒组肺组织中羟脯氨酸含量明显高于对照组,差异有统计学意义(P<0.05).染毒后7、14、21d,与染毒组相比,治疗组羟脯氨酸含量降低,差异有统计学意义(P<0.05).染毒后3d,染毒组和治疗组PT、APTT、Fib、D-dimer明显高于对照组,差异有统计学意义(P<0.05),TT的差异无统计学意义(P>0.05).与染毒组相比,治疗组D-dimer明显降低,差异有统计学意义(P<0.05).染毒后7、14、21d,染毒组和治疗组TT、D-dimer明显高于对照组,差异有统计学意义(P<0.05),PT、APTT、Fib的差异无统计学意义(P>0.05);与染毒组相比,治疗组D-dimer明显降低,差异有统计学意义(P<0.05).光学显微镜下观察,肺组织损伤随染毒时间延长而加重,间隔纤维条索形成,治疗组变化较染毒组为轻.结论 低分子肝素钙联合阿司匹林治疗百草枯中毒能改善实验动物高凝状态,减轻肺损伤.

Objective To establish a model of acute lung injury induced by paraquat poisoning and to observe the effects of anticoagulant therapy on acute lung injury induced by paraquat poisoning.Methods One hundred twenty adult healthy male Wistar rats were randomly divided into three groups:the paraquat poisoning group was exposed intragastricly (IG) to 50 mg/kg paraquat,anticoagulant therapy group was exposed intragastricly (IG) to 50 mg/kg paraquat then administrated subcutaneously with 68 U/kg low molecular heparin calcium 2 times a day and administrated intragastricly with 1.67 mg/kg aspirin one tome a day for 3,7,14 and 21 days,respectively,control group exposed intragastricly to normal saline.After exposure the rats were sacrificed,the venous blood and lung tissues were collected to detect the prothrombin time,activated partial thromboplastin time,fibrinogen,thrombin time and D-dimer in blood and the hydroxyproline in lung tissues,and to examine pathological changes in lung tissues with HE and Masson staining under light microscope.Results At the 3rd,7th,14th and 21st days after exposure,the hydroxyproline contents of lung tissues in paraquat poisoning group and anticoagulation therapy group were significantly higher than those in control group (P<0.05),but the hydroxyproline contents of lung tissues in anticoagulation therapy group were significantly lower than those in paraquat poisoning group (P<0.05).At the 3rd day after exposure,the PT,APTT,Fib and D-dimer levels in paraquat poisoning group and anticoagulation therapy group were significantly higher than those in control group (P<0.05),the D-dimer level of anticoagulation therapy group was significantly lower than that of control group (P<0.05).At the 7th,14th and 21 st days after exposure,the TT and D-dimer levels of paraquat poisoning group and anticongulation therapy group were significantly higher than those of control group (P<0.05),the TT and Ddimer levels of anticoagulation therapy group were significantly lower than those of paraquat poisoning group (P<0.05).The lung injury in paraquat poisoning group increased with exposure period,the lung fibrosis in anticoagulation therapy group was lower than that in paraquat poisoning group.Conclusion Anticoagulation therapy can improve hyper-coagulation state and acute lung injury in rats induced by paraquat poisoning.
