目的 探讨普遍食盐碘化(universal salt iodization,USI)对住院手术患者甲状腺疾病谱的影响.方法 以本院1987-2008年非缺碘地区住院患者的手术病理报告为依据,分析各种病理类型甲状腺疾病的构成比,并对USI前后的甲状腺良、恶性病变发病趋势进行分析.结果 甲状腺癌和结节性甲状腺肿的构成比USI后比USI前明显增加,差异有统计学意义(P<0.01),甲状腺腺瘤的构成比显著下降,差异有统计学意义(P<0.01).结论 非缺碘地区USI实施后,甲状腺病理的组织类型发生显著变化,提示食盐碘化可能是导致甲状腺癌和结节性甲状腺肿增加的重要因素.

목적 탐토보편식염전화(universal salt iodization,USI)대주원수술환자갑상선질병보적영향.방법 이본원1987-2008년비결전지구주원환자적수술병리보고위의거,분석각충병리류형갑상선질병적구성비,병대USI전후적갑상선량、악성병변발병추세진행분석.결과 갑상선암화결절성갑상선종적구성비USI후비USI전명현증가,차이유통계학의의(P<0.01),갑상선선류적구성비현저하강,차이유통계학의의(P<0.01).결론 비결전지구USI실시후,갑상선병리적조직류형발생현저변화,제시식염전화가능시도치갑상선암화결절성갑상선종증가적중요인소.
Objective To investigate effects of pre and post universal salt iodization (USI) on the spectrum of thyroid diseases in inpatients with surgical operations. Methods Pathological reports were collected from the patients with thyroid diseases from non-iodine-deficient region, who received surgery in No. 411Hospital of CPLA from 1987 to 2008. The ratio of each type of thyroid diseases and the tendencies of benign and malignant changes in thyroid diseases were analyzed. Results The ratio of thyroid carcinoma after USI was significantly higher than that before USI (P < 0.01), while the ratio of nodular goiter after USI was also significantly higher than that before USI (P < 0. 01). However, the ratio of thyroid adenoma decreased significantly after USI (P < 0.01). Conclusions Histological types of thyroid diseases from non - iodinedeficient region varied after universal salt iodization: the ratios of thyroid carcinoma and nodular goiter had a tendency to increase, while that of thyroid adenoma tended to decrease.
