目的 研究髓样细胞触发性受体(TREM)-1在急性梗阻性化脓性胆管炎(AOSC)患者外周血单核细胞(PBMC)中表达的规律及其临床意义.方法 分别收集36例AOSC患者(实验组)和40例正常人(对照组)外周血单核细胞,以半定量RT-PCR检测单核细胞中TREM-1 mRNA表达:免疫细胞化学检测单核细胞中TREM-1蛋白表达;ELISA检测患者外周血中TNF-表达水平,免疫透射比浊法测C反应蛋白.结果 对照组和实验组TREM-1 mRNA半定量比值分别为0.4571+-0.053和1.007+-0.252(P<0.05).免疫细胞化学观察结果示实验组TREM-1蛋白阳性表达率与对照组之间差异显著(P<0.01).实验组TNF-和C反应蛋白水平均高于对照组(P<0.01).结论 人TREM-1在急性梗阻性化脓性胆管炎发病初表达明显升高,提示TREM-1在急性梗阻性化脓性胆管炎的发生中可能起重要作用.

Objective To investigate the expression pattern of human triggering receptor expressed on myeloid cells 1 (TREM-1) mRNA in peripheral blood mononuclear cells and its clinical significance in acute obstructive suppurative cholangitis (AOSC). Methods Peripheral blood mononuclear cells were collected from 36 patients with AOSC and 40 healthy adults. TREM-1 mRNA was determined by semi-quantitative RT-PCR, and TREM-1 protein by rmmunocytochemistry. Enzyme-linked immunosorbent assay (TNF-α) was used to detect the level of tumor necrosis factor-α (TNF-α), and imrnunoturbidimetry employed to detect C reactive protein. Results The expression of TREM-1 mRNA relative to β-actin was 1.007±0.252 in patients with AOSC, significantly higher than that in the healthy adults (0.457±0.053, P<0.05). The two groups also showed significantly different TREM protein expression (P<0.01). The AOSC patients exhibited significantly higher levels of TNF-α and C reactive protein than the healthy adults (P<0.01). Conclusion The expression of human TREM-1 in peripheral blood mononuclear cells is up-regulated obviously in early stage of AOSC, probably suggesting an important role of TREM-1 in the development of AOSC.
