目的探讨充血性心力衰竭(CHF)患者血清甲状腺激素(TH)的变化及临床意义。方法用放射免疫法测定30例CHF患者及30例健康对照者血清三碘甲状腺原氨酸(T３)、四碘甲状腺素(T4)、促甲状腺激素(TSH)水平。结果 CHF 患者血清T3、T4、TSH水平分别为0.8±0.28ng/ml、82.21±14.82ng/ml、5.45±2.06mIU/L,其中T３、T４水平明显低于对照组(P＜0.01，P＜0.05);治疗后分别为1.38±0.30ng/ml、95.34±15.82ng/ml、5.35±3.10mIU/L，T3、T4增高与治疗前比较有显著性差异(P＜0.01，P＜0.05)。结论CHF患者体内T3、T4水平下降，且与CHF病情呈负相关，观察TH水平变化对判断CHF患者的病情、疗效及预后有一定临床意义。
목적탐토충혈성심력쇠갈(CHF)환자혈청갑상선격소(TH)적변화급림상의의。방법용방사면역법측정30례CHF환자급30례건강대조자혈청삼전갑상선원안산(T３)、사전갑상선소(T4)、촉갑상선격소(TSH)수평。결과 CHF 환자혈청T3、T4、TSH수평분별위0.8±0.28ng/ml、82.21±14.82ng/ml、5.45±2.06mIU/L,기중T３、T４수평명현저우대조조(P＜0.01，P＜0.05);치료후분별위1.38±0.30ng/ml、95.34±15.82ng/ml、5.35±3.10mIU/L，T3、T4증고여치료전비교유현저성차이(P＜0.01，P＜0.05)。결론CHF환자체내T3、T4수평하강，차여CHF병정정부상관，관찰TH수평변화대판단CHF환자적병정、료효급예후유일정림상의의。
Objectives　In order to determine the levels of thyroid hormone(TH)and its clinical significance in patients with congestive heart failure(CHF).Methods　The triiodothyronine(T3)，thyroxine(T4),thyroid stimulating hormone(TSH)in serum of 30 patients with CHF and 30 normal Contrasts were detected by radioimmunoassay.Results　The concentrations of T3、T4、TSH were 0.8±0.28ng/ml,82.21±14.82ng/ml,5.45±2.06mIU/L respectively in CHF patients and the levels of T3、T4 of patient were lower than that of normal contrasts (P＜0.01，P＜0.05respectively);The concentrations of T3,T4,TSH were 1.38±0.30ng/ml,95.34±15.82ng/ml,5.35±3.10mIU/L,respectively in CHF patients after treatment,the level of T3、T4 were higher than patient of CHF were not treated,there was significant difference (P＜0.01,P＜0.05,respectively).Conclusions　The levels of T3,T4 in CHF patients is decreased,the more severe heart failure,the more obvious decreased of the hormones were observed.These indicate that the alterations of serum thyroid hormone level may be valuable to dentify the deterioration of heart function,the effect of treatment and prognosis in patients with congestive heart failure.
