目的 利用深圳迈瑞BS-400生化分析仪(以下简称BS-400)反应曲线图,分析有关室内质量控制失控、标本测试结果异常以及评价试剂性能的问题.方法 观察、比较甘油三脂(TG)在控与失控反应曲线图,以及γ-谷氨酰基转移酶(GGT)、天门冬氨酸氨基转移酶(AST)等异常结果的反应曲线图,以TG测试为例,结合试剂说明书,观察反应曲线图,分析有关数据及图形.结果 室内质量控制中,TG试剂R1(以下简称R1)失效引起的失控,R1吸光度平均值8.39,正常平均值17,经t检验,和差异具有显著性(P<0.05);测定标本时,GGT=-38 U/L,样本和R2孵育时的吸光度值超过37 331.AST=-25 U/L,曲线图形出现底物耗尽.这些异常结果的反应过程相关吸光度、图形、反应度(R)与正常反应曲线图的比较,有明显的不同;观察TG测试的反应曲线图,其试剂空白值(R0)=221.6、反应曲线平稳.无明显波动,反应4分钟后已趋于平衡,符合试剂说明书有关要求.结论 BS-400反应曲线图,可有效快速地分析有关室内质量控制失控、标本测试结果异常以及评价试剂性能的问题,为解决问题提供了一个极好的线索.

Objective To explore loss of internal quality control(IQC)and abnormal results of specimen tests, and to evaluate reagents property by using response curve on Mindray biochemistry analyzer BS-400. Methods The control and loss of control response cur e for triglyceride(TG), and the abnormal results of response curves for(a)-glutamyl transferase(GGT)and aspartate aminotransfera.se(AST)were observed and compared. Taking TG test as example and referring to reagent instruction manual, we analyzed relevant data and charts. Results In loss of IQC due to inefficacy of TG reagent(R1), the average OD value of R1 was 8.39, while the average normal value was 17, t- test showing a significant difference(P< 0.05). During the incubattion with R1, OD value of the sample exceeded 37 331 when GGT = -38 U/L. As AST = -25 U/L, the response curve resulted in substrate exhaustion. When a specimen test resulted in abnormal data, the OD value, response charts, and responsiveness were significantly different from those on the control response curves. The response curve of TG test was observed; the curve, whose agent blank value was equal to 221.6, was stable without significant fluctuation and it became balanced 4 min after reactions. The test results were in accordance with the reagent manual. Conclusions Response curve on Mindray biochemistry analyzer BS-400 is effective in analyzing loss of IOC and abnormal results of specimen tests and in evaluating reagents' property.
