目的探讨闭角型青光眼患者滤过手术后行白内障超声乳化手术的安全性及临床效果.方法分别对46例(46只眼)闭角型青光眼滤过手术后白内障患者(试验组)54例(54只眼)老年性白内障患者(对照组)行超声乳化白内障吸出人工晶状体植入术,检查术后视力、角膜水肿程度、角膜内皮细胞密度、眼内压,观察手术并发症的发生情况.结果两组患者视力较术前均有显著提高,术后眼压均在正常范围.术中无一例后囊膜破裂,所有患者均植入人工晶状体.试验组术后角膜水肿、前房炎症反应、虹膜的损伤均高于对照组.两组差异有统计学意义(P<0.05).但经过治疗后均好转.角膜内皮细胞损失率两组差异无统计学意义(P>0.05).结论闭角型青光眼滤过术后行白内障超声乳化手术,效果良好.角膜内皮、功能性滤过泡的保护,防止虹膜损伤,术后加强抗炎治疗是保证手术安全的关键.
목적 탐토폐각형청광안환자려과수술후행백내장초성유화수술적안전성급림상효과.방법 분별대46례(46지안)폐각형청광안려과수술후백내장환자(시험조)54례(54지안)노년성백내장환자(대조조)행초성유화백내장흡출인공정상체식입술,검사술후시력、각막수종정도、각막내피세포밀도、안내압,관찰수술병발증적발생정황.결과 량조환자시력교술전균유현저제고,술후안압균재정상범위.술중무일례후낭막파렬,소유환자균식입인공정상체.시험조술후각막수종、전방염증반응、홍막적손상균고우대조조.량조차이유통계학의의(P<0.05).단경과치료후균호전.각막내피세포손실솔량조차이무통계학의의(P>0.05).결론 폐각형청광안려과술후행백내장초성유화수술,효과량호.각막내피、공능성려과포적보호,방지홍막손상,술후가강항염치료시보증수술안전적관건.
Objective To evaluate the effect of phacoemulsification with intraocular lens (IOL)im-plantation after extra-filtering surgery.Methods Phacoemulsification with implantation of IOL was performed on 46 cataract eyes of 46 angle closure glaucoma patients after extra-filtering surgery (Test),and 54 cataract eyes of 54 patients ( Control ).All case received phacoemulsifieation by one surgeon.The visual acuity, corneal edema,endothelial cell loss,IOP and the damage of iris were compared between the two groups.Results All of eyes received IOL without complications such as posterior capsular rupture etc.The postoperative visual acu-ity of all patients improved in different degrees.The corneal edema, anterior chamber rection and damage of iris in test group were heavy than the eyes in the control group, which recovered after therapy.The different of en-dothelial cell loss were not statistically significant.Conclusions The patients obtained perfect result, it is nec-essary to take into account to protect corneal endothelial cell, functional filtering bleb, to reduce the damage of the iris, and to emphasize the treatment after surgery.
