Insulin resistance
目的研究血清抵抗素水平与肥胖、2型糖尿病(T2DM)和胰岛素抵抗(IR)的关系.方法用酶免疫测定法检测31例单纯肥胖、27例T2DM及30名正常人空腹血清抵抗素水平.结果单纯肥胖组、T2DM组及正常对照组空腹抵抗素分别为(41±13)、(43±11)和(53±7)μg/L.单纯肥胖组和T2DM组血清抵抗素浓度均低于正常对照组(P<0.01).空腹血清抵抗素与体质指数(BMI)、胰岛素抵抗指数(HOMA-IR)分别呈负相关(r=-0.292及-0.319,P<0.01),与空腹胰岛素、腰围和体脂百分比(BF%)也呈负相关(r=-0.259,-0.231及-0.239,P<0.05).多元逐步回归分析显示,HOMA-IR为影响抵抗素最为显著的因素(R2=0.086). 结论肥胖及T2DM患者血清抵抗素水平偏低.血清抵抗素与BMI、体脂百分比、腰围、空腹胰岛素和HOMA-IR呈负相关.抵抗素可能在人类IR的发病中起一定的作用.

Objective To investigate the relationship of serum resistin concentration with obesity,type 2 diabetes and insulin resistance Methods The fasting serum resistin concentration was measured by EIA in 31 obese subjects(BMI≥28),27 type 2 diabetes and 30 normal subjects Results The serum resistin levels were (41 29±12 72)、(43 14±10 94) and (53 44±6 97) μg/L in obese,diabetic and normal subjects respectively The resistin concentrations were lower in obese and diabetic subjects than in normal subjects ( P <0 01) Fasting serum resistin was negatively correlated with BMI,HOMA IR and fasting plasma insulin concentrations ( P <0 01) Multiple stepwise regression analysis revealed that the independent variable that was associated with serum resistin levels was HOMA IR ( P <0 01) Conclusions Fasting serum resistin levels are negatively associated with BMI,fasting insulin concentrations as well as insulin resistance Prospective studies are now needed to establish the role of resistin in the pathogenesis of human obesity and insulin resistance
