目的 探讨临床模拟教学系统在解决目前医学人才培养过程中临床实践机会日益减少等困境的可行性和有效性.方法 通过建立功能齐全的临床模拟教学系统,采用4种教学体系和12个实践模块,为医学生和青年教师培训和考核提供高水准的平台.结果 模拟教学系统面向临床医学、麻醉学、预防医学、护理学4个专业的15门课程,3年来开展各专业实习项目124项,完成教学任务9 680学时,有6 015人次接受了训练.培训及录用标准化病人44人,构成内科学、外科学、妇产科学、儿科学、神经病学33个标准化病人典型病例,使用标准化病人930人次,累计3 760学时.2001级和2002级本科生的毕业考试采用客观结构化临床考试.2007年,在系统内对全院住院医师及主治医师300余人的气管插管、心肺复苏等技能进行培训及考核,医师临床抢救能力明显提高.结论 临床模拟教学系统对提高医学生临床动手能力、保证临床教学质量是行之有效的.

Objective The paper is to explore the feasibility and effectiveness of applying clinical simulation in order to solve the problems of lack of practice in the process of training and developing medical professionals.Method A multi-functional clinical simulation teaching system has been developed,which employs four teaching schemes and 12 practice modules,provides a high-standard platform for the training and assessment of medical students and young teachers.Results Clinical simulation teaching system can be applied to 15 courses within the disciplines of clinical medicine,anaesthesia,prevention and nursing.Over the last three years the system has been put into practice in 124 training projects,as well as having been applied to 9 680 hours teaching and training exercises.6 015 people have benefited in developing their clinical skills by undertaking the trainings using the system.Following the training the professional skills were then assessed using 44 standardized patients,who suffered from 33 typical complains in medicine,surgery,gynaecology,paediatrics,and neurology,totalling 3 760 hours.The examinations for the graduates in 2001 and 2002 were carried out by means of objective structure clinical examination in connection with the application of the clinical simulation teaching system.Conclusions Clinical simulation teaching system is a reliable and highly effective tool that Can be successfully employed in developing practical skills for the students and ensuring the quality of teaching and training in clinical medicine practice.
