目的 应用多焦视网膜电图(multifocal electroretinogram,mfERG)观察激光光凝对于视网膜颞上分支静脉阻塞所致黄斑水肿的疗效.方法 对视网膜颞上分支静脉阻塞合并黄斑水肿的病例,进行多焦视网膜电图检查,分析其一阶反应(FOK),N1、P1波振幅密度值及其潜伏期.结果 中心凹处P1波振幅密度在光凝治疗后3个月开始恢复,治疗后6个月明显升高,与光凝治疗前相比差异具有统计学意义(P<0.05、P<0.01);2环P1波潜伏期在光凝治疗后3个月比治疗前有所缩短(P<0.05),到治疗后6个月时改善更加明显,与治疗前相比差异具有统计学意义(P<0.01).颞上象限视网膜P1波振幅密度值在治疗后一个月与治疗前相比下降,其差异具有统计学意义(P<0.05).结论 激光光凝可以有效治疗视网膜分支静脉阻塞所致的黄斑水肿,多焦视网膜电图在疗效评估方面具有重要价值,P1波振幅密度和P1波潜伏期可作为评价疗效的敏感指标.

Objective To observe the therapeutic efficacy of laser photocoagulation for macular edema secondary to superior temporal branch retinal vein occlusion determined by multifocal electroretinogram.Methods Implemented multifocal electroretinogram examination to the patients with macular edema secamplitude densities and latencies ofN1-wave, P1-wave. Results The amplitude density of P1-wave in fovea began to improve at the 3rd month post-treatment and the improvement was more obvious at the 6th month post-treatment with statistically significance (P<0.05, P<0.01). The latency of P1-wave in ring 2 shortened at the 3rd month post-treatment and the improvement was more obvious at the 6th month post-treatment with statistically significance (P<0.05, P<0.01). Compared with pre-treatment value, the amplitude density of P1-wave in superior temporal retina decreased at the 1st month post-treatment with statistically significance (P<0.05).Conclusions Laser photocoagulation can alleviate the macular edema secondary to branch retinal vein occlusion. Multifocal electroretinogram technique has the significant value to evaluate the therapeutic efficacy. The amplitude density and latency of P1-wave may be regarded as sensitive estimate indexes.
