目的 评估无创正压通气对重度哮喘的治疗效果.方法 对比分析无创正压通气与鼻导管吸氧对重度哮喘患者血气分析结果及肺功能的影响.结果 无创正压通气组患者总体有效率(30/36)优于鼻导管吸氧组(14/37),差异有极显著性(JP<0.01);无创正压通气组患者SaO2及PaO2分别为(94.16±4.05)%、(83.01±4.88)%,均显著高于鼻导管吸氧组,差异均有极显著性(P<0.01),其PaCO2下降至(39.42±3.21),亦显著多于鼻导管吸氧组(P<0.01);无创正压通气组患者PEF及FEV1改善程度均显著好于鼻导管吸氧组患者(P<0.01).结论 无创正压通气能迅速改善重度哮喘患者血气分析结果及肺功能.

Objective To evaluate the effects of noninvasive positive pressure ventilation (NIPPV) on severe asthma patients. Methods Comparison of blood gas analysis results and pulmonary function was made between NIPPV group and nasal tube oxygen inhalation group. Results The general effective rate and the improvement of SaO2and PaO2 of NIPPV group were significantly better than those in nasal tube oxygen inhalation group (x2=15.773,P=0.001; t=3.528,P=0.001;t=12.554, P=0.000) while PaCO2 dropped much (t=5.874, P=0.000). The PEF and FEV1 of positive airway pressure ventilation group also improved better than those in nasal tube oxygen inhalation group (r=1.952, P=0.048; t=5.644,P=0.000). Conclusion NIPPV can effectively improve the blood gas analysis results and pulmonary function of severe asthma patients.
