目的：随着CT增强检查的广泛开展,碘对比剂的应用增多,针对有过敏史者在注射对比剂前使用地塞米松预防不良反应的效果进行探讨研究。方法：选择2009年1月至2010年12月间在我院进行CT检查中有过敏史者800例。随机分为试验组400例和对照组400例。地塞米松预防注射对比方法：试验组患者于检查前5min地塞米松10mg＋生理盐水4ml静脉推注;对照组直接注射对比剂,观察两组患者注射对比剂后发生不良反应的程度。结果：两组患者中试验组患者注射对比剂后发生轻度不良反应为2例,预防不良反应的有效率为99.50%;对照组患者注射对比剂后发生不良反应为37例,有效率为90.75%。两组比较有显著差异,有统计学意义（x2=33.01,P〈0.05）。结论：通过临床观察,针对有过敏史者在注射对比剂前使用地塞米松预防不良反应发生是有效的,既能减少对比剂所产生的不良反应,又避免了滥用地塞米松及减少其不良反应发生的概率。
목적：수착CT증강검사적엄범개전,전대비제적응용증다,침대유과민사자재주사대비제전사용지새미송예방불량반응적효과진행탐토연구。방법：선택2009년1월지2010년12월간재아원진행CT검사중유과민사자800례。수궤분위시험조400례화대조조400례。지새미송예방주사대비방법：시험조환자우검사전5min지새미송10mg＋생리염수4ml정맥추주;대조조직접주사대비제,관찰량조환자주사대비제후발생불량반응적정도。결과：량조환자중시험조환자주사대비제후발생경도불량반응위2례,예방불량반응적유효솔위99.50%;대조조환자주사대비제후발생불량반응위37례,유효솔위90.75%。량조비교유현저차이,유통계학의의（x2=33.01,P〈0.05）。결론：통과림상관찰,침대유과민사자재주사대비제전사용지새미송예방불량반응발생시유효적,기능감소대비제소산생적불량반응,우피면료람용지새미송급감소기불량반응발생적개솔。
Objective：To investigate using dexamethasone before injecting the contrast media in patients with allergic history to prevent untoward effect. Methods：From January 2009 to December 2010, 800 patients with allergic history were divided into following two groups：experimental group（400 cases） and control group（400 cases）. The patients in the experimental group were intravenously administered with 10mg dexamethasone and 4 ml physiological saline 5 min before CT scan. Those in control group were only administered with contrast media. The incidence and severity of untoward effect in the two groups were observed and analyzed. Results：There were 2（0.50%） cases with mild untoward effect in the experimental group and effective rate of dexamethsone was 99.50%. There were 37 （9.25%） cases with untoward effect in the control group and effective rate of dexamethsone was 90.75%. The difference of the incidence and the severity between the two groups were statistically significant （P0.05）. Conclusion：Using dexamethasone in patients with allergic history to prevent untoward effect before injecting the contrast media is effective.
