目的 观察程序性死亡因子1(PD1)及其配体PD-L1和PD-L2在糖尿病视网膜病变(DR)患者外周血单个核细胞(PBMCs)中的表达.方法 临床检查确诊为DR的40例患者(DR组)以及同期行体检的健康志愿者20名(对照组)纳入研究.DR组40例患者中,非增生型DR(NPDR)20例,增生型DR(PDR) 20例.抽取两组受检者晨起空腹静脉血2 ml,采用荧光定量聚合酶链式反应(PCR)检测PD-1、PD-L1、PD-L2 mRNA的表达水平.对比分析DR、对照组之间,PDR与NPDR组之间各指标表达水平的差异.结果 荧光定量PCR检测发现,DR组患者PBMCs中PD-1、PD-L1 mRNA相对表达量显著低于对照组,差异有统计学意义(t=-2.060,-2.562;P=0.043,0.013);PD-L2 mRNA相对表达量较对照组无明显变化,差异无统计学意义(t=-0.857,P=0.395).PDR组患者PBMCs中PD-1 mRNA相对表达量较NPDR组有降低趋势,PD-L1、PD-L2 mRNA相对表达量有升高趋势,但差异均无统计学意义(t=-1.335,0.987,0.131;P=0.190,0.334,0.897).结论 DR患者PBMCs中PD-1、PD-L1 mRNA相对表达较正常者低,PD-L2 mRNA相对表达较正常者无明显变化.

목적 관찰정서성사망인자1(PD1)급기배체PD-L1화PD-L2재당뇨병시망막병변(DR)환자외주혈단개핵세포(PBMCs)중적표체.방법 림상검사학진위DR적40례환자(DR조)이급동기행체검적건강지원자20명(대조조)납입연구.DR조40례환자중,비증생형DR(NPDR)20례,증생형DR(PDR) 20례.추취량조수검자신기공복정맥혈2 ml,채용형광정량취합매련식반응(PCR)검측PD-1、PD-L1、PD-L2 mRNA적표체수평.대비분석DR、대조조지간,PDR여NPDR조지간각지표표체수평적차이.결과 형광정량PCR검측발현,DR조환자PBMCs중PD-1、PD-L1 mRNA상대표체량현저저우대조조,차이유통계학의의(t=-2.060,-2.562;P=0.043,0.013);PD-L2 mRNA상대표체량교대조조무명현변화,차이무통계학의의(t=-0.857,P=0.395).PDR조환자PBMCs중PD-1 mRNA상대표체량교NPDR조유강저추세,PD-L1、PD-L2 mRNA상대표체량유승고추세,단차이균무통계학의의(t=-1.335,0.987,0.131;P=0.190,0.334,0.897).결론 DR환자PBMCs중PD-1、PD-L1 mRNA상대표체교정상자저,PD-L2 mRNA상대표체교정상자무명현변화.
Objective To observe the expression of programed death-1 (PD-1) and its ligands including PD-L1 and PD-L2 in peripheral blood mono-nuclear cells (PBMCs) of patients with diabetic retinopathy (DR) patients.Methods Forty patients with DR (DR group) and 20 healthy controls (control group) were included in this study.There were 20 patients with non-proliferative DR (NPDR) and 20patients with proliferative DR (PDR).Peripheral Blood samples were obtained from two groups.Real time polymerase chain reaction (RT-PCR) was used to analyze PD-1,PD-L1,and PD-L2 mRNA expression in PBMCs.The clinical data was analyzed in DR group and controls,also in PDR group and NPDR group.Results The results of RT-PCR showed that the expression of PD-1 and PD-L1 mRNA in DR group were significantly lower than those in the control group (t=-2.060,-2.562; P=0.043,0.013).There was no significant difference in PD-L2 mRNA expression between DR and control group (t=-0.857,P=0.395).Compared with the NPDR group,the lower expression level of PD-1 mRNA and higher expression level of PD-L1 and PD-L2 mRNA in PDR group were observed,but the differences were not statistically significant (t=-1.335,0.987,0.131; P=0.190,0.334,0.897).Conclusion PD-1 and PD-L1 mRNA expression in PBMCs of DR patients is decreased compared with controls,but there are no differences in PD-L2 mRNA expression in them.
