目的 评价组织多肽特异性抗原(TPS)、癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白21-1(CYFRA21-1)联合检测对肺癌的诊断价值.方法 应用电化学发光法(ECLIA)检测经病理确诊的83例肺癌、41例良性肺部疾病和30例正常体检者血清中CEA、NSE、CYFRA21-1和TPS的含量.结果 肺癌组TPS、CEA、NSE、CYFRA21-1测定水平明显高于肺良性病变组和正常对照组,差异有统计学意义(P<0.05或P<0.01);TPS、CEA、NSE、CYFRA21-1对肺癌的敏感性分别为84.34%、51.8l%、43.37%、66.26%,特异性分别为81.93%、79.52%、77.11%、80.72%,诊断符合率分别为83.13%、68.67%、69.88%、79.95%;TPS、CEA、NSE、CY-FRA21-1四项联合检测对肺癌的敏感性、特异性和诊断符合率分别为92.77%、93.97%和95.18%.结论 TPS、CEA、NSE、CYFRA21-1四项肿瘤标记物联合检测可提高肺癌检测的敏感性、特异性和准确性.

Objective To estimate the diagnostic value of combined detection of TPS, CEA,NSE,CYFRA21 - 1 in patients with lung cancer. Methods The content of serum CEA, NSE, CY-FRA21 -1 and TPS were detected with ECLIA. There were three groups: eighty- three pathologically confirmed cases of lung cancer in lung cancer group, 41 benign pulmonary disease cases in benign pulmonary disease group and 30 normal volunteers in normal healthy group. Results The levels of serum TPS, CEA, NSE, CYFRA21 - 1 in lung cancer group were significantly higher than that in benign pulmonary disease group and normal healthy group, with a statistically significant decrease ( P < 0. 05 or P < 0.01 ). The sensitivity of TPS, CEA, NSE, CYFRA21 - 1 to lung cancer was 84. 34%, 51.81%,43.37% and 66.26%, respectively;the specificity was 81.93%, 79.52%, 77.11% and 80. 72%, respectively ,and the coincidental rate of the diagnosis was 83.13%, 68.67%, 69.88% and 79.95%, respectively. While the sensitivity, specificity and the coincidental rate of the diagnosis to lung cancer was 92.77% ,93.97% and 95.18% , respectively, with combined detection of four serum tumor markers (TPS, CEA, NSE, CYFRA21 - 1 ) in lung cancer patients. Conclusions The combined detection of the four serum tumor markers( TPS, CEA, NSE, CYFRA21 - 1 ) can improve the sensitivity, specificity and accuracy in lung cancer detection.
