Real time three dimensional echocardiography%Mitral stenosis.
目的 探讨实时三维超声心动图(RT3DE)评价风湿性二尖瓣狭窄瓣口面积的可行性与准确性.方法 26例拟行二尖瓣置换的风湿性二尖瓣狭窄患者,术前应用二维面积法和实时三维超声测量狭窄二尖瓣口面积(MVA),分别获得MVA_(2DE)和MVA_(3DE).术后对二尖瓣标本采用照相法测量瓣口面积(MVA_(OP)),与上述2种方法测值进行比较.结果 MVA_(OP)与MVA_(3D)之间差异无统计学意义(P>0.05),但与MVA_(2D)间差异有统计学意义(P<0.05).MVA_(2D)、MVA_(3D)与MVA_(OP)相关系数分别为0.96和0.97 (P<0.05).MVA_(2D)、MVA_(3D)与MVA_(OP)间均有良好的一致性.结论 实时三维超声心动图能够简便、快速、准确测量风湿性二尖瓣口狭窄面积.

Objective To determine the feasibility and accuracy of real time three dimensional echocardiography(RT3DE)in evaluation of mitral valve area(MVA)in patients with rheumatic mitral stenosis.Methods MVA was measured using two dimensional echocardiography(MVA_(2DE))and RT3DE(MVA_(3DE))in 26 patients.The stenotic mitral specimens were acquired in mitral valve replacement surgery,and MVA_(OP) were calculated with photoshop and Matlab software.MVA_(OP) were compared with MVA_(2DE) and MVA_(3DE) respectively.Results There was no significant difference between MVA_(OP) and MVA_(3D)( P> 0.05),but statistical difference was found between MVA_(OP) and MVA_(2D)( P<0.05).MVA_(2D),MVA_(3D) and MVA_(OP) correlation coefficient were 0.96 and 0.97 respectively(P<0.05).Bland-Altman analysis demonstrated that both MVA_(2D) and MVA_(3D) had good agreement with MVA_(OP).Conclusions RT3DE can provide an easy,feasible and accurate method for quantifying MVA in patients with rheumatic mitral stenosis.
