目的:通过测量收缩功能正常的冠心病患者择期经皮冠状动脉介入治疗(PCI)术前、术后血氨基末端脑钠肽前体(NT-proBNP)浓度的变化,探讨择期PCI对NT-proBNP浓度的影响及可能机制.方法:行择期PCI的冠心病患者60例,分别检测PCI术前、术后24 h内血NT-proBNP、肌酸激酶同工酶(CK-MB)及肌钙蛋白-T(cTn-T),同时分析PCI术前血NT-proBNP浓度与冠状动脉病变间的关系.结果:择期PCI术后血NT-proBNP浓度降低,lg(NT-proBNP)从术前的(2.68±0.46)ng/L降低到(2.53±0.41)ng/L,两者比较差异有统计学意义(P<0.01),而择期PCI术前术后CK-MB、cTn-T浓度无明显变化;择期PCI术前血NT-proBNP浓度随着冠状动脉受累支数、冠状动脉狭窄程度的加重而升高.结论:收缩功能正常的冠心病患者择期PCI可降低血NT-proBNP浓度,其机制可能与心肌缺血改善有关.

Objective:To investigate the impact and the mechanism of elective PCI on the serum level of NT-proBNP in patients with coronary heart disease and normal systolic function.Method:The NT-proBNP, CK-MB and cTn-T concentrations before and after elective PCI were assigned in 60 patients diagnosed as coronary heart disease, the relationship between baseline NT-proBNP level and the severity of coronary disease was also analysed. Result:The concentration of NT-proBNP declined after elective PCI, from (2.68±0.46)pg/ml before the procedure reduced to (2.53±0.41)pg/ml after that,the difference was statistically significant (P<0.01), but the levels of CK-MB and cTn-T did not change in the procedure; and the baseline level of NT-proBNP was dependent on the severity of coronary disease. Conclusion:In patients with CHD and normal systolic function, elective PCI can reduce the level of NT-proBNP, maybe due to the amelioration of myocardial ischemia by angipalsty.
