目的 探讨实施人文关怀护理前后唇腭裂整复术婴幼儿围术期焦虑程度的变化.方法 选择2008年5月~2009年6月行唇腭裂整复术的患儿42例为护理组,实施人文关怀护理;选择2007年3月~2008年4月行唇裂整复术患儿38例为对照组,实施常规护理采用改良耶鲁围术期焦虑量表(mYPAS)于术前访视(T1)、人手术室(T2)、麻醉诱导时(T3)、术后24 h(T4)4个时间点测量患儿的焦虑值,以诱导期合作评分(ICC)观察诱导时患儿合作程度,以调查表调查家属的满意度.结果 与T1时间点相比,两组患儿T2及T3两个时间点焦虑评分明显升高(P<0.01),T4回到术前值;两组比较,护理组患儿在T3时其焦虑评分明显降低(P<0.01),同时护理组患儿合作评分也明显降低(P<0.01);护理组患儿家属对医务人员满意率明显高于对照组,差异有显著性(P<0.01).结论 采用人文关怀护理能明显的减轻唇腭裂整复术患儿围术期的焦虑程度.
目的 探討實施人文關懷護理前後脣腭裂整複術嬰幼兒圍術期焦慮程度的變化.方法 選擇2008年5月~2009年6月行脣腭裂整複術的患兒42例為護理組,實施人文關懷護理;選擇2007年3月~2008年4月行脣裂整複術患兒38例為對照組,實施常規護理採用改良耶魯圍術期焦慮量錶(mYPAS)于術前訪視(T1)、人手術室(T2)、痳醉誘導時(T3)、術後24 h(T4)4箇時間點測量患兒的焦慮值,以誘導期閤作評分(ICC)觀察誘導時患兒閤作程度,以調查錶調查傢屬的滿意度.結果 與T1時間點相比,兩組患兒T2及T3兩箇時間點焦慮評分明顯升高(P<0.01),T4迴到術前值;兩組比較,護理組患兒在T3時其焦慮評分明顯降低(P<0.01),同時護理組患兒閤作評分也明顯降低(P<0.01);護理組患兒傢屬對醫務人員滿意率明顯高于對照組,差異有顯著性(P<0.01).結論 採用人文關懷護理能明顯的減輕脣腭裂整複術患兒圍術期的焦慮程度.

Objective To investigate the effects of nursing techniques with humanistic nursing on perioperative anxiety in infants and young children. Methods Compared with 38 cases with eheilopalatognathus (C group), 42 cases with eheilopalatognathus (H group)were performed by humanistic nursing. The anxiety level were tested by mYPAS (modified Yale re operative anxiety scale) at 4 time points:the day before operation, walking into the operating room, anesthesia induction and the day after operation. The cooperative levels during the anesthesia induction were tested by ICC (induction compliance checklist) . Results In both groups, there was significantly higher anxiety level when walking into the operating room and during the anesthesia induction than that at the day before operation, but the children in H group showed lower anxiety level and better cooperation than that in C group during the anesthesia induction. Conclusions Combining nursing techniques with humanistic nursing can have a good effect to lower the anxiety level in infancy and children.
