目的:定量分析肾肿瘤的血流显像状况。方法:用彩色多普勒能量图检测42例肾肿瘤血流,在电子微量注射泵控制下经患者周围静脉2分钟内匀速注入Levovist 2.5g,采用彩色直方图软件,通过测定肿瘤总切面积与其内血流显示面积之比(BFAR),定量分析肿瘤的血流显像状况。结果:不论是良性还是恶性肿瘤,Levovist均能显著增强瘤内血流显像,两者BFAR值无显著差异,但分析注射造影剂后的历时变化发现:良性和恶性肾肿瘤血流显像增强开始时间分别为37、25秒(中位数,下同。P=0.040),增强持续时间分别为372、550秒,(P=0.002),恶性肿瘤内血流具有快速出现,延缓消失的特点。诊断性试验以增强持续时间400秒为截断点,诊断恶性肿瘤的准确性为82.7%。结论:借助彩色多普勒能量图良好的血流显像能力和无创性的声学造影技术,进而对造影剂注射的剂量、速度和时间作定量控制,对造影结果作定量分析,有可能更客观地揭示肾肿瘤的血流显像特征,有助于病变性质的鉴别诊断。

Objective:To evaluate the blood flow imaging status of renal tumors.Methods:Color Doppler energy(CDE)was used to assess tumor blood flow in 42 patients with renal tumors,in which 2.5g Levovist was injected evenly via peripheral vein over 2 minutes under control of electronic pump.Using histogram software,blood flow area ratio(BFAR)of renal tumors was measured.Results:Levovist remarkably enhanced the intra-tumoral blood flow imaging in either benign or malignant tumors,with no significant difference in BFAR.Analysys of its changes with time revealed that the starting time of blood flow imaging enhancement was 37s and 25s in benign and malignant tumors,respectively (P=0.04),and the lasting time of enhancement was 372s and 550s,respectively(P=0.002).It demonstrated that blood flow in malignant tumor was characterized with swift appearance and delayed disappearance.Diagnostic trial showed that the lasting time of enhancement of 400s was the cut-off point,with an accuracy of 82.7% for diagnosis of malignant tumor.Conclusions:Armed with good blood flow imaging capacity of CDE and noninvasive echo-angiography,further more,based on quantitative control of contrast dose,injection speed and time,and quantitative analysis of the imaging,it was possible to more objectively reveal the blood flow imaging features of renal tumors,and help the differential diagnosis of the disease.
