目的研究老年高血压病人血清Ⅲ型前胶原(PCⅢ)的浓度变化及临床意义。方法应用放免法测定33例Ⅰ、Ⅱ期老年高血压患者血清PCⅢ的含量,以20例血压正常的同龄健康人作对照;同时对病人行心脏彩色多普勒检查,测定左室重量(LVM)及二尖瓣口舒张早期、舒张晚期最大血流速度(VE、VA)。结果老年高血压组与对照组比较,血清PCⅢ浓度增高(P<0.001)。在老年高血压病组,血清PCⅢ与VE/VA呈负性相关(r=-0.403 4,P<0.05);与LVM无相关关系(r=0.165 8,P>0.05)。结论在老年高血压病人中,血清PCⅢ参与了高血压的发生、发展过程;使心肌硬度增加,心肌纤维化,影响心脏的舒张功能。

목적연구노년고혈압병인혈청Ⅲ형전효원(PCⅢ)적농도변화급림상의의。방법응용방면법측정33례Ⅰ、Ⅱ기노년고혈압환자혈청PCⅢ적함량,이20례혈압정상적동령건강인작대조;동시대병인행심장채색다보륵검사,측정좌실중량(LVM)급이첨판구서장조기、서장만기최대혈류속도(VE、VA)。결과노년고혈압조여대조조비교,혈청PCⅢ농도증고(P<0.001)。재노년고혈압병조,혈청PCⅢ여VE/VA정부성상관(r=-0.403 4,P<0.05);여LVM무상관관계(r=0.165 8,P>0.05)。결론재노년고혈압병인중,혈청PCⅢ삼여료고혈압적발생、발전과정;사심기경도증가,심기섬유화,영향심장적서장공능。
Objective To study the changes in concentration of serum procollagen type Ⅲ (PCⅢ) and the correlations between the PCⅢ and cardiac function parameters(VE/VA),left ventricular mass(LVM) in aged essential hypertensive patients.Methods The concentration of PCⅢ was measured by radioimmunoassay in 33 aged essential hypertensive patients and 20 normotensive subjects.At same time,two-dimensional M-mode Doppler ultrasound examination was made to determine patient's LVM,maximal early transmittal flow velocity(VE) and maximal late transmittal flow velocity(VA).Results The concentration of serum PCⅢ increased in aged essential hypertensives than that in normotensives (P<0.001).And in aged essential hypertensives,an inverse correlation was found between PCⅢ and VE/VA(P<0.05);but there is no relationship between PCⅢ and LVM(P>0.05).Conclusions PCⅢ involved in the occurrence and development of the essential hypertension,which made myocardium fibrosis and affected cardiac diastolic function.
