本文报告100例生后48小时内的新生儿的肛管直肠测压结果,91例出现内括约肌松驰反射,一周内99例出现此反射.扩张直肠后肛管压力下降的平均幅度随直肠充气量的增加而增大.因此肛管直肠测压诊断新生儿先天性巨结肠症是可行的.

Anorectal manometry was carried out in 100 unselected neonates.In all babies,a first manometry was performed within 48 hours after birth.In 9 cases with a negative reflex and in 17 with a positive reflex in the first manometry,a second manometry was done on the third day after birth.A third was done on the seventh day after birth in 3 cases with a negative reflex in the previous 2 manometries.Among the 100 neonates,91 showed a relaxation of the internal anal sphincter in response to rectal distension within 48 hours after birth.All the 100 had a relaxation on the seventh day except a premature.The average static pressure and the average contractive frequency were 11.6mmHg and 9.0c/m in thearial canal,6.5mmHg and 5.3c/m in the rectum,7.1mmHg and 4.9c/m in the sigmoid respectively.The average value of pressure-drop in anal canal showed a positive correlation with the rectal distention(P＜0.001).These findings suggest that the relaxation reflex of internal anal sphincter has been well established in seven days after birth in all normal neonates,and that the anorectal manometry is valuable for the diagnosis of Hirschsprun's disease in neonatal period.
