目的 探讨三维动态增强MR血管成像所显示门静脉高压患者侧支循环的分布情况.方法 选取我院检查的69例门静脉高压患者,并进行门静脉系统的3D DCE MRA检查,对减影后的影像学资料及MI P重建图像进行分析,统计侧支循环的分布情况.结果 69例门静脉高压患者中,食管和食管周围静脉曲张45例,胃底静脉曲张47例,奇/半奇静脉曲张35例;胃左静脉曲张34例,胃后静脉曲张11例,胃-肾分流13例;脾周静脉曲张14例,脾-肾分流者4例;附脐静脉曲张21例;性腺静脉曲张17例.结论3D DCE MRA不仅能发现门静脉高压患者的曲张的侧支循环,还可以明确血管起止、走行和毗邻关系.

목적 탐토삼유동태증강MR혈관성상소현시문정맥고압환자측지순배적분포정황.방법 선취아원검사적69례문정맥고압환자,병진행문정맥계통적3D DCE MRA검사,대감영후적영상학자료급MI P중건도상진행분석,통계측지순배적분포정황.결과 69례문정맥고압환자중,식관화식관주위정맥곡장45례,위저정맥곡장47례,기/반기정맥곡장35례;위좌정맥곡장34례,위후정맥곡장11례,위-신분류13례;비주정맥곡장14례,비-신분류자4례;부제정맥곡장21례;성선정맥곡장17례.결론3D DCE MRA불부능발현문정맥고압환자적곡장적측지순배,환가이명학혈관기지、주행화비린관계.
Objective To explore the distributions of the collateral circulation in portal hypertension patients displayed by 3D-DCE-MRA.Methods 69 patients with portal hypertension who had undergone hepatic portal vein 3D-DCE-MRA in our department were selected.These image data were analyzed after subtracting and MIP reconstructing.Then added up the distribution of collateral circulation.Results Among the 69 cases,there were 45 cases with esophageal varix,47cases with gastric fundus varix,35 cases with azygos or hemiazygos varix,34 cases with left gastric varix,11 cases with posterior gastric varix,13 cases with gastric renal shunt,14 cases with renal varix,4 cases with spleen renal shunt,21 cases with paraumbilical veins rarix,and 17 cases with gonad varix.Conclusion 3D-DCE-MRA not only can discover the varix collateral circulation,but also is able to display the veins from origin to the end and the relations between the veins and their surrounding structure more clearly.
