目的:研究Nd:YAG激光照射对未处理过的和用超声波或手工器械刮治过的牙周病患牙根面的影响。方法:选择因严重牙周病而须拔除的单根牙18颗,随机平均分为6组,在临床麻醉下分别先后作如下治疗:超声波刮治+激光照射;激光照射+超声波刮治;手工器械刮治+激光照射;激光照射+手工器械刮治;超声波刮治;手工器械刮治。然后拔除患牙作扫描电镜观察。结果:单纯用超声波或手工器械刮治过的牙根表面均有明显的涂层,而用激光(20pps、2.0W、3min)照射,无论是在刮治前还是刮治后,均有助于根面涂层的去除,但激光对牙根表面结构有不同程度的影响。结论:先用Nd:YAG激光照射、再行龈下刮治,牙根表面的涂层被清除,而结构改变最小。

목적:연구Nd:YAG격광조사대미처리과적화용초성파혹수공기계괄치과적아주병환아근면적영향。방법:선택인엄중아주병이수발제적단근아18과,수궤평균분위6조,재림상마취하분별선후작여하치료:초성파괄치+격광조사;격광조사+초성파괄치;수공기계괄치+격광조사;격광조사+수공기계괄치;초성파괄치;수공기계괄치。연후발제환아작소묘전경관찰。결과:단순용초성파혹수공기계괄치과적아근표면균유명현적도층,이용격광(20pps、2.0W、3min)조사,무론시재괄치전환시괄치후,균유조우근면도층적거제,단격광대아근표면결구유불동정도적영향。결론:선용Nd:YAG격광조사、재행간하괄치,아근표면적도층피청제,이결구개변최소。
Objective:This study was to evaluate the effect of Nd:YAG laserirradiation on untreated and subgingival scaled by manual or ultrasonic instrumentation periodontally involved root surfaces.Methods:18 single rooted human teeth with sever periodontitis were assigned randomly to 6 groups.These groups separately were treated in clinical anesthesia by ultrasonic scaling+irradiation,irradiation+ultrasonic scaling,manual scaling+irradiation,irradiation+ manual scaling,ultrasonic scaling,manual scaling.Then these teeth were extracted and SEM was performed.Results:It was observed that surface smear layer was present on the root surfaces of teeth that were scaled by ultrasonic or manual instrumentation only;Regardless of treatment sequence,laser irradiation for 3 minutes using energy settings of 2.0W at 20pps was able to remove the smear layer,but the root surfaces of teeth exhibited some degree of laser-induced alteration.Conclusions:Treated by laser for 3 minutes at settings of 2.0W and 20pps followed by subgingival scaling, surface smear layer could be removed and there was minimal change on the root surfaces.
