目的观察重度烧伤患者中后期全身和创面局部肉芽组织中的微量元素Fe、Cu、Zn、Se、Mn的含量变化,探讨微量元素对创面愈合的影响.方法收集38例重度烧伤患者伤后4～9周未愈创面肉芽组织及全血标本,利用等离子体发射法测定其Fe、Cu、Zn、Se、Mn的含量,并与正常人体皮肤和血标本进行对照.结果伤后4～9周血液和创面肉芽组织中的Zn、Se、Mn含量均逐渐下降；Cu在血液中的含量接近正常,在肉芽组织中的含量逐渐上升,伤后8～9周时Cu含量为(1.31±0.21) μg/g湿重,高于人体正常皮肤中的含量,差异有统计学意义(P＜0.05).创面局部Zn和Se的含量变化与血液中的变化显著相关(rzn =0.747,P＜0.05；rse=0.852,P＜0.05).结论微量元素Fe、Cu、Zn、Se、Mn在全身和创面局部肉芽组织中的变化不尽相同,局部Zn、Se含量的变化受全身含量的影响,不同微量元素的不同变化可能是导致创面愈合延迟的重要因素.
목적 관찰중도소상환자중후기전신화창면국부육아조직중적미량원소Fe、Cu、Zn、Se、Mn적함량변화,탐토미량원소대창면유합적영향.방법 수집38례중도소상환자상후4～9주미유창면육아조직급전혈표본,이용등리자체발사법측정기Fe、Cu、Zn、Se、Mn적함량,병여정상인체피부화혈표본진행대조.결과 상후4～9주혈액화창면육아조직중적Zn、Se、Mn함량균축점하강；Cu재혈액중적함량접근정상,재육아조직중적함량축점상승,상후8～9주시Cu함량위(1.31±0.21) μg/g습중,고우인체정상피부중적함량,차이유통계학의의(P＜0.05).창면국부Zn화Se적함량변화여혈액중적변화현저상관(rzn =0.747,P＜0.05；rse=0.852,P＜0.05).결론 미량원소Fe、Cu、Zn、Se、Mn재전신화창면국부육아조직중적변화불진상동,국부Zn、Se함량적변화수전신함량적영향,불동미량원소적불동변화가능시도치창면유합연지적중요인소.
Objective To explore the changs of trace elements in local wound of the severe burn patients at the middle and late stage postburn in order to investigate the effects of trace elements on wound healing.Methods Granulation tissues obtained from burn wound of 38 severe burn patients,all patients were divided into 3 groups,i.e.group A (n =12,during 4-5 weeks postburn),group B (n =16,during 6-7 weeks postburn),group C (n =10,during 8-9 weeks postburn).Furthermore,normal skins harvested form 10 volunteers performed operation,were taken as control (group D).The contents of trace elements were detected by atomic absorption assay method.Results The contents of Zn,Se,Mn in granulation tissues were decreased gradually during 4-9 weeks postburn,and lower than that in normal skins( P ＜0.05 ).The contents of Cu increased gradually,and higher than that in normal skins since 8 weeks after burn injury.Meantime,the contents of Fe,have no obvious change compared to that in normal skins.Conclusion Every trace element developes differently during the time of wound healing,and the change of each element may greatly affect the wound healing.
