Chronic schistosomiasis Gut-associated cathodic antigen Praziquantel Immunodiagnosis
目的 观察不同亚治疗剂量吡喹酮对慢性血吸虫病患者血清血吸虫肠相关趋阴极抗原(GACA)水平的影响.方法 Kato-Katz法粪检确定108例慢性血吸虫病患者,随机分成4组,分别给予口服5mg/kg、10mg/kg、20mg/kg剂量吡喹酮和安慰剂.采用单克隆抗体酶联免疫吸附试验(McAb-DotEUSA)分别检测服药前、后2、8、24和48小时患者血清GACA水平.服药前、后检出率变化采用γ2检验.结果 3个实验组服药后2小时检测血清GACA水平,平均较服药前上升3个滴度,后渐下降;至服药后48小时,血清GACA水平仍高于服药前1个滴度.血清GACA检测敏感性,可由服药前的82.7%提高到服药后2小时的100.0%(P<0.05).对照组48小时观察期内,血清GACA水平波动在1个滴度范围内,检测敏感性无明显变化(P>0.05).结论 口服不同亚治疗剂量吡喹酮均能有效升高慢性血吸虫病患者血清血吸虫GACA水平,并显著提高血清GACA检测敏感性.

목적 관찰불동아치료제량필규동대만성혈흡충병환자혈청혈흡충장상관추음겁항원(GACA)수평적영향.방법 Kato-Katz법분검학정108례만성혈흡충병환자,수궤분성4조,분별급여구복5mg/kg、10mg/kg、20mg/kg제량필규동화안위제.채용단극륭항체매련면역흡부시험(McAb-DotEUSA)분별검측복약전、후2、8、24화48소시환자혈청GACA수평.복약전、후검출솔변화채용γ2검험.결과 3개실험조복약후2소시검측혈청GACA수평,평균교복약전상승3개적도,후점하강;지복약후48소시,혈청GACA수평잉고우복약전1개적도.혈청GACA검측민감성,가유복약전적82.7%제고도복약후2소시적100.0%(P<0.05).대조조48소시관찰기내,혈청GACA수평파동재1개적도범위내,검측민감성무명현변화(P>0.05).결론 구복불동아치료제량필규동균능유효승고만성혈흡충병환자혈청혈흡충GACA수평,병현저제고혈청GACA검측민감성.
Objective To observe the effect of different subclinical doses of praziquantel on the serum gut-associated cathodic antigen(GACA)levels in the patients with chronic schistosomiasis.Methods One hundred and eight c8se8 of patients with chronic schistosomiasis diagnosed by Kato-Katz method were divided into 4 groups,which were given praziquantel at the dosage of 5mg/kg,lOmg/kg,20mg/kg and placebo respectively.The serum samples were collected and detected by McAb-Dot-ELISA before treatment and at 2h,8h,24h and 48h after treatment.Statistic analysis was γ2 test.Results The serum GACA levels of all three experimental groups at 2h after treatment were higher than that before treatment by 3 titers.However,the titer was decreased gradually after 2h of the thereapy.Till 48h after treatment the serum GACA levels were still 1 titer higher than that before treatment.The positive detection rate of serum GACA was raised from 82.7%before treatment to 100.0%at 2h after treatment(P<0.05).Comparatively,there were no obvious changes on the serum GACA levels and the positive detection rate in the control group (P>0.05)during the Same period of observation.Conclusion Different subclinical doses of praziquantel can increase the serum GACA levels as well as the sensitivity of GACA detection effectively in the patients with chronic schistosomiasis significantly.
