目的 探讨2型糖尿病(T2DM)患者尿Nephrin排泄的变化以及吡格列酮干预对其的影响.方法 将98例T2DM患者随机分为磺脲类药物干预组(DS组)、吡格列酮干预组(DP组),观察治疗前后相关代谢指标及尿Nephrin排泄的变化,选择同期健康体检人群49名为正常对照(NC)组.结果 DP组、DS组患者FPG、HbA1c、尿白蛋白(UA1b)/尿肌酐(UCr)比值较NC组明显升高(P<0.01).NC组尿中未检测到Nephrin;DP组干预后尿Nephrin/UCr比值明显下降(P<0.01),DS组干预前后尿Nephrin/UCr比值的差异无统计学意义(P>0.05).结论 吡格列酮可降低T2DM患者尿Nephrin排泄,对肾小球足细胞有保护作用.

Objective To investigate the change of urinary Nephrin excretion and the effect of pioglitane on the change in the patients with T2DM. Method We examined various clinical indexes in normal control group, sulfonylurea group and pioglitazone group. Results The levels of FPG, HbA1c, and the ratio of UAlb to UCr were higher in type 2 diabetic patients than in normal control at basal line (P<0.05). There was no urinary Nephrin detected in normal control group, but Nephrin was found in the urine of diabetics. The ratio of urinary Nephrin to UCr was positively related with the ratio of UAlb to UCr. They all decreased after 12 weeks of therapy. The differences in the two ratios showed no statistical significance between pre-and post-therapy of sulfonylurea, but showed obvious statistical significance in pioglitazone group. Conclusion Pioglitazone could reduce the increased urinary Nephrin excretion and provide some podocyte protection in T2DM.
