背景:健脾补肾中药对实验性脾虚证有较好的防治作用,环核苷酸及甲状腺激素水平的变化是否与药物作用机制有关?目的:观察健脾补肾方药对实验性脾虚证大鼠环核苷酸及甲状腺激素水平的影响,探讨该方防治脾虚证的作用机制及脾与肾相互关系的意义.设计:随机对照的实验研究.地点和材料:实验地点为广州中医药大学核医学教研室.实验动物为雄性SD大鼠,体质量190~230 g,3月龄,普通级,由中山大学实验动物中心提供(动物合格证号为2001A032).干预:将大鼠随机分为正常对照组(n=10)、脾虚模型组(n=12),健脾补肾方高剂量组(n=12)和健脾补肾方低剂量组(n=12),采用大黄复制脾虚证动物模型,分别给予生理盐水、大黄、健脾补肾方煎液(高剂量:22 g/kg,低剂量:11g/kg)灌胃.各组动物于最后1次给药后1 h从眼球取血,观察各组大鼠血浆环磷酸腺苷(cyclic adenosine monophos-phate,cAMP),环磷酸鸟苷(cyclic guanosine monophosphate,cGMP),及甲状腺素(thyroxine,T4)、三碘甲腺原氨酸(thiiodothyronine,T3)、促甲状腺素(thyroid stimulating hormone,TSH)水平的变化.主要观察指标:各组大鼠cAMP,cGMP,T4,T3,TSH水平.结果:脾虚模型组大鼠血浆cAMP含量[(30.29±6.55)nmol/L]及cAMP/cGMP(2.94±1.13)比值均比正常对照组升高,T3[(1.55±:0.23)nmol/L],T4[(67.73±14.17)nmol/L]含量比正常对照组下降,差异有显著性意义(q=5.87,7.91,5.61,P均<0.01).高剂量健脾补肾方药能降低实验性脾虚证大鼠体内cAMP含量[(20.81±3.75)nmol/L]及cAMP/cGMP比值(1.34±0.52),提高T3[(2.19±0.38)nmol/L],T4[(89.70±21.27)nmol/L]含量,与脾虚模型组比较,差异有显著性意义(q=6.49,6.56,7.86,4.87,P均<0.01);升高脾脏、胸腺质量比值,减轻肾脏组织质量.结论:健脾补肾中药可以消除大黄的药物影响,防止出现脾虚症候,其作用机制与调节cAMP、cGMP及甲状腺激素水平有关.

BACKGROUND: Although jianpi bushen recipe(JPBS) acted satisfactorily on the treatment of experimental spleen deficiency, whether did the changes in levels of cyclic nucleotide and thyroid hormone be related to the mechanism of the recipe?OBJECTIVE: To observe the effect of JPBS on the level of cyclic adenosine monophosphate (cAMP), cyclic guanosine monophosphate (cGMP) and the thyroid hormone in rats with experimental spleen deficiency and probe into the mechanism of the recipe on the treatment, as well as the interrelationship between spleen and kidney.DESIGN: Randomized controlled experiment.SETTING and MATERIALS: The experiment was performed in the Department of Nucleus Medical of Guangzhou University of Traditional Chinese Medicine. Male SD rats, weighing 190 to 230 g, at the age of 3 months, nor mai class, were employed in the experiment, which were provided by the Experimental Animal Center of Sun Yat-sen University(Qualified No. 2001A032).INTERVENTIONS: The rats were divided at random into normal control group( n = 10), model group of spleen deficiency( n = 12), JPBS high dosage group( n = 12) and JPBS low dosage group( n = 12) . The model of spleendeficiency was duplicated with dahuan. Gastric infusion was done with physiological saline, dahuan and JPBS decoction(high dosage: 22 g/kg, low dosage: 11 g/kg). Blood was collected from eyeball at the last medication in every group to observe the changes in levels of cAMP, cyclic cGMP, thyroxine(T4), thiiodothyronine(T3), and thyroid stimulating hormone(TSH) of rats in every group.MAIN OUTCOME MEASURES: Levels of cAMP, cGMP, T4, T3 and TSH of rats in every group.RESULTS: In model group of spleen deficiency, the content of serum cAMP [ (30. 29±6.55)nmol/L] and cAMP/cGMP(2.94±1.13)were higher than the normal control group; the contents of T3[(1.55±0.23 nmol/L) and T4[ (67.73 ± 14. 17 nmol/L)were reduced compared with the normal control,indicating significant difference( q = 5.87, 7.91, 5.61, P < 0.01) . JPBS recipe of high dosage reduced the content of cAMP[ (20. 81 ± 3.75 nmol/L)and cAMP/cGMP was(1.34±0.52) and increased the contents of T3[ (2.19±0.38 nmol/L) and T4 [ (89.70 ± 21.27) nmol/L] in the rats with experimental spleen deficiency, compared with the model of spleen deficiency, indicating significant difference( q = 6.49, 6.56, 7.86,4. 87; P <0.01) and it also increased the mass ratio of spleen and thymus and decreased the mass of kidney tissues.CONCLUSION: JPBS recipe acted on reducing the medical influence of dahuan and preventiong the symatoms of spleen deficiency.The mechanism of it was related to the levels of cAMP ,cGMP and thyroid hormone.
