小儿肺部感染结核后可出现纵隔淋巴结之瘤样增生或液化坏死,并能造成严重气管压迫及致命性呼吸困难,本文报告4例,均经紧急剖胸而获得治愈.在危重情况下应除外喉梗阻,气管异物.支气管淋巴结结核是小儿肺结核原发综合征的一部分,在有效的抗痨治疗下如仍有明显的、进行性的气管压迫症状,有必要考虑行手术切除.

Four cases of serious respiratory distress caused by mediastinal and tracheobrochial lymphnodes are presented. All of them were treated with emergency thoracotomy. The symptoms and signs of tracheobronchial obstruction include: orthopnea, cyanosis, anoxic restlessness and stridulous breathing. Unconsciousness was found in one patient. The massive nodular enlargement of mediastinal nodes was found in all patients during emergency thoracotomy. The decompressing procedure included the removal of large masses and evacuation of large abscess, The symptom of tracheobronchial compression was relieved in all patients after surgery. All petients recovered well after anti-TB therapy. An early surgical intervention for such kind of patient seems to be very rational.
