目的运用一种新型螺钉(子母螺钉)内固定治疗MasonⅡ型桡骨头骨折,并从功能恢复及影像学角度评价其疗效.方法 2010年10月至2012年2月,对9例MasonⅡ型桡骨头骨折采用切开复位子母螺钉内固定.术后采用Broberg-Morrey评分标准,结合影像学对肘关节功能进行评定.结果所有患者均获得7～ 13个月的随访,平均9.4个月.9例骨折均愈合,时间为1.5～3.2个月,平均2.3个月.Broberg-Morrey评分平均为93.2(87～ 98);优5例,良4例；优良率为100％.所有患者均恢复以前工作,时间平均为15周.1例桡神经深支损伤的患者于术后2.5个月完全恢复.术后骨折均得到解剖复位,无一例发生内固定断裂、骨折复位丢失、感染及异位骨化.结论切开复位、子母螺钉内固定治疗MasonⅡ型桡骨头骨折疗效满意,功能恢复好,并发症少.
목적 운용일충신형라정(자모라정)내고정치료MasonⅡ형뇨골두골절,병종공능회복급영상학각도평개기료효.방법 2010년10월지2012년2월,대9례MasonⅡ형뇨골두골절채용절개복위자모라정내고정.술후채용Broberg-Morrey평분표준,결합영상학대주관절공능진행평정.결과 소유환자균획득7～ 13개월적수방,평균9.4개월.9례골절균유합,시간위1.5～3.2개월,평균2.3개월.Broberg-Morrey평분평균위93.2(87～ 98);우5례,량4례；우량솔위100％.소유환자균회복이전공작,시간평균위15주.1례뇨신경심지손상적환자우술후2.5개월완전회복.술후골절균득도해부복위,무일례발생내고정단렬、골절복위주실、감염급이위골화.결론 절개복위、자모라정내고정치료MasonⅡ형뇨골두골절료효만의,공능회복호,병발증소.
Objective To evaluate the functional and radiographic results of treating Mason type Ⅱ radial head fractures with open reduction and internal fixation with a new type of screw (child-mother screw,CMS).Methods From October 2010 to February 2012,9 cases of Mason type Ⅱ radial head fracture were treated with open reduction and internal fixation with the child-mother screw.The functional outcomes were assessed using Broberg-Morrey scoring system.Radiographically,osteoarthritis or heterotopic ossification was investigated.Results All the patients were follow-up for a mean time of 9.4 months (range,7 to 13 months).Bony healing was achieved in all the patients,within a mean of 2.3 months (range,1.5 to 3.2 months).The mean BrobergMorrey score was 93.2 (range,87 to 98),leading to excellent results in 5 patients and good results in 4 patients.The overall excellent and good rate was 100％.All the patients returned to pre-injury work in a mean of 15 weeks.Complete functional recovery was observed 2.5 months postoperatively in the one case with accompanying radial nerve deep branch injury.Anatomical reduction of the fracture was achieved in all the patients.No complications of infection,implant breakage and heterotopic ossification occurred.Conclusion Open reduction and internal fixation with child-mother screw has good effect,satisfactory functional recovery and few complications in the treatment of Mason type Ⅱ fractures of the radial head.
