目的 研究同型半胱氨酸与糖尿病肾病的发病关系及叶酸、甲钴胺降低血浆Hcy水平的效果.方法 (1)血浆Hcy与糖尿病肾病关系研究:选取健康者30例为对照组,2型糖尿病患者50例,其中糖尿病肾病患者25例,测定3组受检者SBP、DBP及血浆Hcy、FBG、TG、HDL、LDL并分析其余糖尿病肾病的相关性.(2)糖尿病肾病患者Hcy水平干预研究:将60例2型糖尿病患者随机分为A、B两组各30例,A组患者未予干预.B组患者给予口服叶酸及甲钴胺干预治疗8周,比较干预前后血浆Hcy水平的变化.结果 非DN组和DN组患者Hcy水平均显著高于对照组(P<0.05)且DN组显著高于非DN组(P<0.01),多因素分析得Hcy是糖尿病肾病的独立危险因素.干预后患者血浆Hcy水平显著下降且显著低于对照组(P< 0.01).结论 Hcy是糖尿病肾病的独立危险因素,且叶酸联合甲钴胺治疗可显著降低2型糖尿病患者血浆Hcy水平.
目的 研究同型半胱氨痠與糖尿病腎病的髮病關繫及葉痠、甲鈷胺降低血漿Hcy水平的效果.方法 (1)血漿Hcy與糖尿病腎病關繫研究:選取健康者30例為對照組,2型糖尿病患者50例,其中糖尿病腎病患者25例,測定3組受檢者SBP、DBP及血漿Hcy、FBG、TG、HDL、LDL併分析其餘糖尿病腎病的相關性.(2)糖尿病腎病患者Hcy水平榦預研究:將60例2型糖尿病患者隨機分為A、B兩組各30例,A組患者未予榦預.B組患者給予口服葉痠及甲鈷胺榦預治療8週,比較榦預前後血漿Hcy水平的變化.結果 非DN組和DN組患者Hcy水平均顯著高于對照組(P<0.05)且DN組顯著高于非DN組(P<0.01),多因素分析得Hcy是糖尿病腎病的獨立危險因素.榦預後患者血漿Hcy水平顯著下降且顯著低于對照組(P< 0.01).結論 Hcy是糖尿病腎病的獨立危險因素,且葉痠聯閤甲鈷胺治療可顯著降低2型糖尿病患者血漿Hcy水平.
목적 연구동형반광안산여당뇨병신병적발병관계급협산、갑고알강저혈장Hcy수평적효과.방법 (1)혈장Hcy여당뇨병신병관계연구:선취건강자30례위대조조,2형당뇨병환자50례,기중당뇨병신병환자25례,측정3조수검자SBP、DBP급혈장Hcy、FBG、TG、HDL、LDL병분석기여당뇨병신병적상관성.(2)당뇨병신병환자Hcy수평간예연구:장60례2형당뇨병환자수궤분위A、B량조각30례,A조환자미여간예.B조환자급여구복협산급갑고알간예치료8주,비교간예전후혈장Hcy수평적변화.결과 비DN조화DN조환자Hcy수평균현저고우대조조(P<0.05)차DN조현저고우비DN조(P<0.01),다인소분석득Hcy시당뇨병신병적독립위험인소.간예후환자혈장Hcy수평현저하강차현저저우대조조(P< 0.01).결론 Hcy시당뇨병신병적독립위험인소,차협산연합갑고알치료가현저강저2형당뇨병환자혈장Hcy수평.
Objective To research the relationship between Hcy and DN and observe the effect of folic acid and methylcobalamin in the level of Hcy.Methods The blood samples of normal people,DM patients and DN patients were measured to compare the levels of Hcy,FBG,TG,HDL,LDL.60 T2DM patients were divided into group A and group B,30 patients in group B were treated with folic acid and methylcobalamin for 8 weeks.Results The level of Hcy of DM group and DN group were all significantly higher than that of control group,and the level of Hcy of DN group was higher than that of DM group(P< 0.05).Multivariate analysis showed Hcy level was the independent risk factor of DN.The Hcy level of group B was significantly higher than that of group A after treatment(P < 0.01).Conclusion Hcy is the independent risk factors of DN; folic acid and methylcobalamin are remarkable effective in reducing Hcy level.
