目的 探讨门冬氨酸钾镁对心脏手术患者术后心律失常的预防和治疗作用.方法 采用多中心、随机、平行、对照的方法,在术后(包括术后当天)连续3 d分别给予试验组门冬氨酸钾镁、对照组氯化钾注射液适量静脉滴注,以维持血钾的正常水平.术后连续心电监测,记录患者心律、电解质及肾功能等指标.结果 手术当天、术后第1天、术后第2天两组患者的补钾量比较,差异均有统计学意义(P<0.05);术前、术后试验组患者的血钾水平与对照组比较,差异均无统计学意义(P>0.05),在术前、术后即刻、术后当晚,试验组患者的血镁水平高于对照组,差异有统计学意义(P<0.05);术后试验组患者心房纤颤、房性期前收缩、室性期前收缩的发生率均低于对照组,心脏功能也较对照组改善明显.结论 手术前后及时给予门冬氨酸钾镁补充钾、镁离子,与单纯补充氯化钾相比,能更有效地维持患者血钾水平、改善心脏功能并显著减少心律失常的发生.
目的 探討門鼕氨痠鉀鎂對心髒手術患者術後心律失常的預防和治療作用.方法 採用多中心、隨機、平行、對照的方法,在術後(包括術後噹天)連續3 d分彆給予試驗組門鼕氨痠鉀鎂、對照組氯化鉀註射液適量靜脈滴註,以維持血鉀的正常水平.術後連續心電鑑測,記錄患者心律、電解質及腎功能等指標.結果 手術噹天、術後第1天、術後第2天兩組患者的補鉀量比較,差異均有統計學意義(P<0.05);術前、術後試驗組患者的血鉀水平與對照組比較,差異均無統計學意義(P>0.05),在術前、術後即刻、術後噹晚,試驗組患者的血鎂水平高于對照組,差異有統計學意義(P<0.05);術後試驗組患者心房纖顫、房性期前收縮、室性期前收縮的髮生率均低于對照組,心髒功能也較對照組改善明顯.結論 手術前後及時給予門鼕氨痠鉀鎂補充鉀、鎂離子,與單純補充氯化鉀相比,能更有效地維持患者血鉀水平、改善心髒功能併顯著減少心律失常的髮生.
목적 탐토문동안산갑미대심장수술환자술후심률실상적예방화치료작용.방법 채용다중심、수궤、평행、대조적방법,재술후(포괄술후당천)련속3 d분별급여시험조문동안산갑미、대조조록화갑주사액괄량정맥적주,이유지혈갑적정상수평.술후련속심전감측,기록환자심률、전해질급신공능등지표.결과 수술당천、술후제1천、술후제2천량조환자적보갑량비교,차이균유통계학의의(P<0.05);술전、술후시험조환자적혈갑수평여대조조비교,차이균무통계학의의(P>0.05),재술전、술후즉각、술후당만,시험조환자적혈미수평고우대조조,차이유통계학의의(P<0.05);술후시험조환자심방섬전、방성기전수축、실성기전수축적발생솔균저우대조조,심장공능야교대조조개선명현.결론 수술전후급시급여문동안산갑미보충갑、미리자,여단순보충록화갑상비,능경유효지유지환자혈갑수평、개선심장공능병현저감소심률실상적발생.
Objective To Investigate the prevention effects of potassium and magnesium aspartate on arrhythmia after cardiac surgery.Methods A multi-center,randomized,parallel and controlled method was used to give test group an appropriate intravenous infusion of potassium-magnesium aspartate and control group potassium chloride injection to maintain normal level of blood potassium.Postoperative continuous ECG monitoring was conducted and heart rate,electrolytes and renal function were recorded.Results There was significant difference in potassium supplement between 2 groups on surgery day,days 1,2 after operation (P<0.05),but there was not in blood potassium level (P>0.05).The level of magnesium ions was higher in test group than in control,the difference was significant (P<0.05).The incidence of fibrillation,atrial premature contraction,ventricular extrasystole and ventricular fibrillation was lower in test group than in control,and cardiac function improved more significantly.Conclusion Timely supplement of potassium-magnesium aspartate is more effective to maintain blood potassium,improve cardiac function and reduce significantly the incidence of arrhythmias than pure potassium chloride supplement.
