目的 观察贝前列素钠对老年2型糖尿病肾病(DN)患者的临床疗效,并观察患者血浆纤维蛋白原(FN)及尿白蛋白排泄率(UAER)的变化.方法 测定27例老年2型无肾病糖尿病患者、48例老年2型临床糖尿病肾病患者血浆FN水平.将48例老年2型糖尿病肾病患者随机分为常规治疗组24例和贝前列素钠治疗组24例,观察两组治疗前后FN、UAER的变化.结果 老年2型糖尿病肾病患者较老年2型无肾病糖尿病患者血浆FN 水平明显升高(P<0.01);与对照组比较,应用贝前列素钠可使糖尿病肾病患者血浆FN水平明显降低(P<0.01),UAER明显降低(P<0.05).结论 FN水平升高在糖尿病肾病的发病中发挥重要作用.贝前列素钠治疗可纠正糖尿病肾病患者血浆FN平衡失调,减少UAER,促进病情恢复.

목적 관찰패전렬소납대노년2형당뇨병신병(DN)환자적림상료효,병관찰환자혈장섬유단백원(FN)급뇨백단백배설솔(UAER)적변화.방법 측정27례노년2형무신병당뇨병환자、48례노년2형림상당뇨병신병환자혈장FN수평.장48례노년2형당뇨병신병환자수궤분위상규치료조24례화패전렬소납치료조24례,관찰량조치료전후FN、UAER적변화.결과 노년2형당뇨병신병환자교노년2형무신병당뇨병환자혈장FN 수평명현승고(P<0.01);여대조조비교,응용패전렬소납가사당뇨병신병환자혈장FN수평명현강저(P<0.01),UAER명현강저(P<0.05).결론 FN수평승고재당뇨병신병적발병중발휘중요작용.패전렬소납치료가규정당뇨병신병환자혈장FN평형실조,감소UAER,촉진병정회복.
Objective To observe the clinical curative effect of beraprost sodium on elderly patients with type 2 diabetic nephropathy (DN),and to observe the changes of plasma fibrinogen (FN)and urinary albumin excretion rate (UAER).Methods The plasma FN levels of 27 senile type 2 diabetes patients without nephropathy,48 elderly patients with type 2 diabetic nephropathy were measured.Forty-eight cases of elderly patients with type 2 diabetic nephropathy were divided into two groups,the conventional treatment group (24 cases) and beraprost sodium treatment group (24 cases),the changes of levels of FN and UAER in the two groups before and after treatment were observed.Results The plasma FN levels in the elderly patients with type 2 diabetic nephropathy were significantly elevated than those in elderly patients with type 2 diabetes without nephropathy (P <0.01).Compared with the control group,the plasma levels of FN and UAER were significantly reduced in patients with type 2 diabetic nephropathy by beraprost sodium (P < 0.05).Conclusions The level of FN increase at the onset of diabetic nephropathy and play an important role in the pathogenesis of diabetic nephropathy.Beraprost sodium in treatment of patients with diabetic nephropathy can correct the plasma FN imbalance,and it can reduce UAER and promote recovery.
