目的 探讨冰桶及空气波压力治疗仪对膝关节置换术后患者肢体肿胀、疼痛状况的影响,以及预防下肢深静脉血栓(DVT)构成的临床效果.方法 将50例双膝关节置换术后患者随机分为实验组及对照组各25例,两组均采用常规预防方法,实验组应用冰桶及空气波压力治疗仪,2次/d,50 min/次,12 d为1疗程;对照组未使用治疗仪.对两组患者DVT发生率及肿胀、疼痛方面进行观察比较.结果 治疗2周后两组患者肢体肿胀、肢体疼痛情况比较,差异均有统计学意义(P<0.01).治疗组治疗1周后肢体重度肿胀、疼痛及DVT的发生率明显低于对照组(P<0.01).结论 冰桶及空气压力波治疗仪能明显减轻膝关节置换术后患者下肢肿胀、疼痛并减少了DVT的发生率,对早期DVT的预防有明显的作用.URBANKOVA研究[1]发现,间断加压器械预防DVT和不预防的相比较,DVT发生率降低了60%.2009年9月~2010年12月,我科使用冰桶及空气波压力治疗仪对膝关节置换术后患者进行预防性治疗,临床效果比较满意.

Objective To study the effect of ice bucket and airwave pressure therapeutic apparatus on patients' body swelling and pain after knee arthroplasty,as well as the clinical effects of prevention from lower limbs deep vein thrombosis (DVT).Methods A total of 50 patients with double knees arthroplasty were randomly divided into control group and experimental group with 25 cases in each.Two groups all took routine preventive measures.The experimental group took ice bucket and airwave pressure therapeutic apparatus,2 times per day,50min per times,12 days for one course of treatment.The control group did not use the apparatus.Two groups of patients'DVT incidence,swelling and pain were observed and compared.Results Two weeks after treatment,the differences of two groups of patients' body swelling and pain were significant statistically (P <0.01 ).One week after the degree of body swelling,pain and the incidence of DVT in experimental group were obviously lower than those in control group ( P < 0.01 ).Conclusions Ice bucket and air wave pressure therapeutic apparatus can significantly relieve body swelling,pain and the incidence of DVT.
