目的 探讨胰岛素样生长因子Ⅱ(IGF-Ⅱ)及碱性成纤维细胞生长因子(bFGF)在肾上腺疾病发生过程中所起的作用及临床意义.方法 收集标本分为肾上腺嗜铬细胞瘤组、皮髓质增生组、皮质腺瘤组、正常肾上腺组织组各15例,采用免疫组织化学二步法检测各组IGF-Ⅱ及bFGF蛋白在各组的表达情况.结果 IGF-Ⅱ及bFGF蛋白表达水平在四组中表达差异均有统计学意义(均P<0.05),表达率从高到低依次为人肾上腺嗜铬细胞瘤组、肾上腺皮髓质增生组、肾上腺皮质腺瘤组,而正常肾上腺组皆地低表达或不表达.各组中IGF-Ⅱ及bFGF蛋白表达均呈正相关(均P<0.05).前三组IGF-Ⅱ及bFGF蛋白阳性细胞面积与血压均存在正相关关系(均P<0.05).结论 IGF-Ⅱ及bFGF蛋白在人肾上腺皮髓质增生症、皮质腺瘤,特别是嗜铬细胞瘤的发生发展中及其高血压的形成可能起重要作用,同时二者之间存在协同作用.

목적 탐토이도소양생장인자Ⅱ(IGF-Ⅱ)급감성성섬유세포생장인자(bFGF)재신상선질병발생과정중소기적작용급림상의의.방법 수집표본분위신상선기락세포류조、피수질증생조、피질선류조、정상신상선조직조각15례,채용면역조직화학이보법검측각조IGF-Ⅱ급bFGF단백재각조적표체정황.결과 IGF-Ⅱ급bFGF단백표체수평재사조중표체차이균유통계학의의(균P<0.05),표체솔종고도저의차위인신상선기락세포류조、신상선피수질증생조、신상선피질선류조,이정상신상선조개지저표체혹불표체.각조중IGF-Ⅱ급bFGF단백표체균정정상관(균P<0.05).전삼조IGF-Ⅱ급bFGF단백양성세포면적여혈압균존재정상관관계(균P<0.05).결론 IGF-Ⅱ급bFGF단백재인신상선피수질증생증、피질선류,특별시기락세포류적발생발전중급기고혈압적형성가능기중요작용,동시이자지간존재협동작용.
Objective To investigate the expressions and significance of insulin-like growth factor Ⅱ and basic fibroblast growth factor in adrenal disease. Methods The experiment was divided into groups of adrenal pheochromocytoma,corticomedullary hyperplasia,cortical adenoma,normal adrenal tissue.Every group was 15 cases.Expression of IGF-Ⅱ and bFGF were detected by immunohistochemistry in adrenal disease. Results There was a significant difference among the four groups(all P<0.05).IGF-Ⅱ and bFGF expression was higher in adrenal pheochromocytoma tissue than that of corticomedullary hyperplasia and cortical adenoma.In the normal adrenal gland group was low expression or no expression.In each group,IGF-Ⅱ and bFGF protein expression was positively correlated(all P<0.05).There also was a positive correlation between IGF-Ⅱ and bFGF positive cell and blood pressure in adrenal pheochromocytoma,corticomedullary hyperplasia and cortical adenoma(all P<0.05). Conclusion IGF-Ⅱ and bFGF protein were closely related to the occurrence and development of human adrenal corticomedullary hyperplasia,cortical adenoma,pheochromocytoma,and played a role in hypertension caused by them.At the same time there was a synergistic effect between the IGF-Ⅱ and bFGF protein in adrenal disease.
