Ultrasonic misdiagnosis%Sillicon oil endotamponade
目的 探讨眼科超声在硅油眼检查中诊断视网膜脱离的误诊率.方法 对208例215只眼玻璃体视网膜疾病患者行玻璃体切割及硅油填充术,在硅油填充术前、硅油填充期、硅油取出术后分别进行B超检查.结果 术前超声诊断符合率为100%,硅油填充期超声诊断的准确率为98.60%.误诊率为1.40%.取油后超声诊断的准确率为96.28%,误诊率为3.72%.结论 超声诊断对视网膜疾病的准确率较高,但是也无法完全避免误诊、漏诊情况,尤其是硅油填充时,为避免误诊和漏诊,超声检查时,不但要注意按操作时反复多轴向多切面扫描,仔细观察强回声带运动情况,判断光带与球壁、晶状体、玻璃体的关系,还要密切结合临床其他有关资料,进行综合分析.

목적 탐토안과초성재규유안검사중진단시망막탈리적오진솔.방법 대208례215지안파리체시망막질병환자행파리체절할급규유전충술,재규유전충술전、규유전충기、규유취출술후분별진행B초검사.결과 술전초성진단부합솔위100%,규유전충기초성진단적준학솔위98.60%.오진솔위1.40%.취유후초성진단적준학솔위96.28%,오진솔위3.72%.결론 초성진단대시망막질병적준학솔교고,단시야무법완전피면오진、루진정황,우기시규유전충시,위피면오진화루진,초성검사시,불단요주의안조작시반복다축향다절면소묘,자세관찰강회성대운동정황,판단광대여구벽、정상체、파리체적관계,환요밀절결합림상기타유관자료,진행종합분석.
Objective To evaluate the misdiagnosis rate of ultrasonic before and after using common sillicon oil as an endotamponade.Methods All 215 eyes of 208 patients aged 12 to 80 years old received vitrectomy and silicon oil tamponade were included.Every patient was examined by the ultrasonic examination before,during the silicon oil endotamponade and after the silicon oil removal.Results The preoperative diagnose accordance rate of ultrasonic diagnosis was 100%.During the period of the silicon oil filled the diagnose accordance rate of B-ultrasonic diagnosis was 98.60% and the misdiagnosis rate was 1.40%.After silicon oil removal the diagnose accordance rate of ultrasonic diagnosis was 96.28% and the misdiagnosis rate was 1.40%.Conclusions The diagnose accordance rate of ultrasonic is high for vitreous and retinal diseases.The value of the ultrasonic diagnosis for vitreous and retina diseases is higher.However misdiagnosis can't be completely avoided.To avoid the misdiagnosis and missed diagnosis,we must be careful during the examine procedure of the ultrasonic diagnosis and must combined with clinic tightly.
