目的 探讨食源性脑寄生虫病的临床特点、诊断及治疗方法.方法 分析8例脑寄生虫病患者的临床表现、流行病学资料、实验室检查、影像学表现及治疗效果.结果 8例患者中,脑囊虫病4例、脑裂头蚴病2例、脑肺吸虫病2例;患者中3例为本地人,5例为外地流动人员,均有生食史.出现头痛5例、癫痫发作4例、发热4例、乏力4例;8例患者头颅CT或MRI检查均有异常表现,血清特异性抗体IgG检测均阳性.用吡喹酮进行抗寄生虫治疗;治后随访,5例症状完全缓解,3例部分缓解.结论 影像学特征、饮食史是食源性脑寄生虫病临床诊断的重要依据;血清中特异性抗体检测对脑寄生虫病诊断和鉴别诊断具有重要意义;抗寄生虫药物治疗对脑囊虫病、肺吸虫病具有较好的疗效.
目的 探討食源性腦寄生蟲病的臨床特點、診斷及治療方法.方法 分析8例腦寄生蟲病患者的臨床錶現、流行病學資料、實驗室檢查、影像學錶現及治療效果.結果 8例患者中,腦囊蟲病4例、腦裂頭蚴病2例、腦肺吸蟲病2例;患者中3例為本地人,5例為外地流動人員,均有生食史.齣現頭痛5例、癲癇髮作4例、髮熱4例、乏力4例;8例患者頭顱CT或MRI檢查均有異常錶現,血清特異性抗體IgG檢測均暘性.用吡喹酮進行抗寄生蟲治療;治後隨訪,5例癥狀完全緩解,3例部分緩解.結論 影像學特徵、飲食史是食源性腦寄生蟲病臨床診斷的重要依據;血清中特異性抗體檢測對腦寄生蟲病診斷和鑒彆診斷具有重要意義;抗寄生蟲藥物治療對腦囊蟲病、肺吸蟲病具有較好的療效.

Objective To study the clinical features,diagnostic methods and treatment effects of food-borne parasitic encephalopathy.Methods The clinical manifestation,epidemiological data,immunological tests and imageological characteristics of 8 cases were analysed.Results Of the 8 cases,4 cases were diagnosed as cysticercosis,2 as sparganosis and 2 as paragonimiasis.Three cases were native,5 cases were outsiders.They all had raw food history.There were 5 cases suffered from headache,4 from epilepsy,4 from fever,4 from fatigue.All the patients were abnormalities on cranial CT of MRI,and the results of specific antibody IgG were positive.After treated with praziquantal,5 cases were completely relieved with no symptoms and 3 cases were partially relieved.Conclusions Imaging features,dietary history are the main evidences to the diagnosis,and the immunological detection of specific antibodies could assist differentiation.Anti-parasite chemical-therapy shows effective to cysticercosis and paragonimiasis.
