目的探讨食源性脑寄生虫病的临床特点、诊断及治疗方法.方法分析8例脑寄生虫病患者的临床表现、流行病学资料、实验室检查、影像学表现及治疗效果.结果 8例患者中,脑囊虫病4例、脑裂头蚴病2例、脑肺吸虫病2例；患者中3例为本地人,5例为外地流动人员,均有生食史.出现头痛5例、癫痫发作4例、发热4例、乏力4例；8例患者头颅CT或MRI检查均有异常表现,血清特异性抗体IgG检测均阳性.用吡喹酮进行抗寄生虫治疗；治后随访,5例症状完全缓解,3例部分缓解.结论影像学特征、饮食史是食源性脑寄生虫病临床诊断的重要依据；血清中特异性抗体检测对脑寄生虫病诊断和鉴别诊断具有重要意义；抗寄生虫药物治疗对脑囊虫病、肺吸虫病具有较好的疗效.

Objective To study the clinical features,diagnostic methods and treatment effects of food-borne parasitic encephalopathy.Methods The clinical manifestation,epidemiological data,immunological tests and imageological characteristics of 8 cases were analysed.Results Of the 8 cases,4 cases were diagnosed as cysticercosis,2 as sparganosis and 2 as paragonimiasis.Three cases were native,5 cases were outsiders.They all had raw food history.There were 5 cases suffered from headache,4 from epilepsy,4 from fever,4 from fatigue.All the patients were abnormalities on cranial CT of MRI,and the results of specific antibody IgG were positive.After treated with praziquantal,5 cases were completely relieved with no symptoms and 3 cases were partially relieved.Conclusions Imaging features,dietary history are the main evidences to the diagnosis,and the immunological detection of specific antibodies could assist differentiation.Anti-parasite chemical-therapy shows effective to cysticercosis and paragonimiasis.
