目的 了解胎盘早剥产前漏诊的原因.方法 对2003～2008年间胎盘早剥产前未确诊的55例(观察组)进行分析,并与同期产前确诊为胎盘早剥的49例(对照组)比较.结果 观察组发病诱因以胎膜早破、妊娠合并症或并发症、子宫异常为主,临床表现主要为产程中胎心监护异常或宫缩过强,早剥面积及总出血量均明显低于对照组.对照组发病诱因以妊娠期高血压疾病、外伤、胎膜早破为主.结论 症状不典型、产程进展过快及临床医生经验不够是胎盘早剥漏诊的主要原因,对产妇及围产儿有较大影响.
目的 瞭解胎盤早剝產前漏診的原因.方法 對2003～2008年間胎盤早剝產前未確診的55例(觀察組)進行分析,併與同期產前確診為胎盤早剝的49例(對照組)比較.結果 觀察組髮病誘因以胎膜早破、妊娠閤併癥或併髮癥、子宮異常為主,臨床錶現主要為產程中胎心鑑護異常或宮縮過彊,早剝麵積及總齣血量均明顯低于對照組.對照組髮病誘因以妊娠期高血壓疾病、外傷、胎膜早破為主.結論 癥狀不典型、產程進展過快及臨床醫生經驗不夠是胎盤早剝漏診的主要原因,對產婦及圍產兒有較大影響.
목적 료해태반조박산전루진적원인.방법 대2003～2008년간태반조박산전미학진적55례(관찰조)진행분석,병여동기산전학진위태반조박적49례(대조조)비교.결과 관찰조발병유인이태막조파、임신합병증혹병발증、자궁이상위주,림상표현주요위산정중태심감호이상혹궁축과강,조박면적급총출혈량균명현저우대조조.대조조발병유인이임신기고혈압질병、외상、태막조파위주.결론 증상불전형、산정진전과쾌급림상의생경험불구시태반조박루진적주요원인,대산부급위산인유교대영향.
Objective To study the causes of misdiagnosis of atypical abruptio placentae. Methods The 55 cases of antepartum misdiagnosed abruptio placentae from 2003 to 2008 were analyzed retrospectively,and compared with 49cases of antepartum diagnosed group. Results In antepartum misdiagnosed group,predisposing factors of the disease were mainly preterm rupture of membrane, complications of pregnancy or compication in pregnancy and abnormity of uterus, and clinical appearances were mainly abnormality of the fetal heart rate monitoring in labor or uterus's excessive contraction. The area of abruptio placentae and total amount of bleeding were significantly lower than those of antepartum diagnosed group. Predisposing factors of the comparison group were hypertensive disorders in pregnancy, traumas and preterm rupture of membrane. Conclusion The atypical clinical appearances, short active phase of labor and physicians' shortage of clinical experiences are the cause of antepartum misdiagnosed, which coffected maternity patients and perinatal newborns very much.
