目的 探讨体外循环心脏瓣膜置换术时机体葡萄糖耐受能力的变化。方法 对56例心脏瓣膜置换病人术前、术中、术后12 h,24 h,48 h的血糖水平进行了动态观察。结果 体外循环心脏瓣膜手术期间血糖明显增高,增高的程度与转流时间相关。结论 手术过程中有效控制病人的血糖浓度,可以避免围手术期并发症的发生。

목적 탐토체외순배심장판막치환술시궤체포도당내수능력적변화。방법 대56례심장판막치환병인술전、술중、술후12 h,24 h,48 h적혈당수평진행료동태관찰。결과 체외순배심장판막수술기간혈당명현증고,증고적정도여전류시간상관。결론 수술과정중유효공제병인적혈당농도,가이피면위수술기병발증적발생。
Objective To probe the changes in blood glucose toleration during heart-valve replacement operation under cardiopulmonary bypass (CPB). Method Dynamic surveys of blood glucose levels of 56 cases were carried out 12 hrs, 24 hrs and 48 hrs before the operations, during them and after them respectively, and were measured by spectrophotometry. Result The glucose levels in blood from patients with heart-valve metathesis operation were 9.2±0.51 μmol/L(CPB 60 min) and 12.4±0.62 μmol/L(CPB 120 min) respectively, which were significantly higher than normal (5.3±0.47)μmol/L and 5.6±0.53 μmol/L(P<0.05). The glucose levels rose noticeably and the extent was in relation to CPB time. Conclusion An effective control over the glucose concentration helps to prevent complication during peri-operations.
